We are seeking a full-time Registered Nurse (RN) Clinical Trainer to facilitate personal care training, clinical training and competency evaluations to new hires in our Woodbury Office. In this position, you will have the opportunity to collaborate with clinicians, clients, their families, and CareAparent’s Home Health interdisciplinary and leadership teams. We maintain a commitment to creativity and idea sharing about improving client care and preparing our clinicians and healthcare professionals for their career in home health.

Salary Range: $70,500.00 to $93,600.00

Position Duties:

Train new hires in company policies and procedures, evaluating engagement and participation
Conduct shadow shifts, unannounced visits, and attend weekly meetings as requested
Collaborate with manager to ensure effective performance, development, and retention programs
Conduct initial home health care OASIS assessments for comprehensive care, including Starts of Care, Resumptions of Care, and Recertifications, maintaining 32-40 productivity units
Perform skilled nursing tasks as requested under the direction of the Clinical Manager and CareAparent scheduling
Oversee and supervise Licensed Practical Nurse (LPN) visits and ensure plan of care is being followed
Coordinate, oversee, and supervise on-site CNA / PCA / HHA visits and ensure plan of care is being followed
Report changes in client condition to physicians and Clinical Manager
Prepare appropriate medical documentation of client assessments and visits using CareAparent EMR platforms
Perform other nursing functions as may fall state and federal nursing guidelines, CMS, and the Joint Commission
Develop a plan of care based on physician orders and initial client assessment and continue to monitor, revise, and update based on ongoing assessments or changes of condition
Facilitate referrals to interdisciplinary team (Physical Therapy, Occupational Therapy, Aroma Therapy, Speech Therapy and Falls Prevention)
Perform medication set-ups as needed or required under the direction of the Clinical Manager and CareAparent scheduling
Be self-directed to manage an effective and productive caseload of patients and clients.

Qualifications:

Graduate from an accredited and approved nursing program
Valid Minnesota RN License, maintained annually
Minimum of one (1) year of relevant home care, assisted living and/or senior care experience
Supervisory/management and training experience preferred
Must possess positive, enthusiastic attitude, good people skills, and professional appearance.
Ability to work independently without constant supervision.
Strong presentation skills.
Experience using a variety of multimedia training platforms and methods.
Ability to evaluate and research and implement training options and alternatives.
Proficient knowledge and use of Microsoft Office Suite and related program software, most importantly Microsoft PowerPoint.
Basic technology and computer skills.
