  • Assist with managing budget for defined area; provides costeffective use of resources, to provide care.
  • Participates in and represents the Department of Surgery and Nursing at corporate administrative professional forums and meetings.
  • Ensures compliance with standards of regulatory agencies, including The Joint Commission on the Accreditation of Healthcare Organizations and New York State and the New York City Department of Health.

Describe responsibility:

  • Provide primary care for the General Surgery/Neurosurgery Clinic under the supervision of collaborating physician.
  • Collaborate with a multidisciplinary program to meet the needs of individual patients and the needs of the overall General Surgery/Neurosurgery population.
  • Orders customary laboratory, radiological and diagnostic studies per practice protocols, differentiates between normal/abnormal findings and follows protocols for abnormal findings, which may include referral to consulting physicians per customary pattern of the attending MD or with the collaborating physician.
  • Formulates the plan of care, along with the attending physician, patient and family, based on expected goals of care.
  • Works collaboratively with the attending physician, consulting physician(s), and other disciplines to identify, develop, and implement an appropriate plan of care that maximizes individual patient/family preference and enhances quality, access, and cost effective outcomes.
  • Evaluates treatment and health care plans for effectiveness and modifies per clinical standards and practice protocols.
  • Educates patient and family to facilitate and promote healthy adjustment to new or changed health patterns.
  • Participate in weekly staff meetings, regular supervision meetings and in case management meetings as needed.
  • Participate in staff development.
  • Establish and maintain professional contact with the Bronx Care Health System, community medical providers and other community organizations promoting health and social wellbeing.
  • Perform other activities as shall be deemed appropriate in assuring services to patients and to support the positions responsibilities.

  • Provides anticipatory guidance re: nutrition, dental health, contraception, smoking, alcohol, drug use hygiene.
  • Contributes to the health education of individuals and groups and applies methods designed to increase each person's motivation to assume responsibility for his own health care.
  • Plans and implements routine care practices in collaboration with medical and other health professionals.
  • Conduct comprehensive home visits to patients who are referred to the asthma referral home visit program
  • Identifies and helps in the management of social, economic and environmental influences affecting the patient and his family.
  • Assesses and manages common acute and chronic illnesses in accordance with protocols.
  • Prescribes selective medications under standing orders of the Physician.
  • Assists in coordinating and planning referral for patients with complex medical problems.
  • Assists in the establishment of policies and procedures relevant to comprehensive, longitudinal care.
  • Participates in outreach programs and screening programs for case identification and followup and community health education.
  • Within the team, uses principles of group development to better understand her partners and to make the team more efficient.
