JOB PURPOSE:
The Clinical Nurse Practitioner Manager (CNPM) will collaborate with community primary care physicians to ensure effective oversight and coordination of participant care within an Interdisciplinary Team framework.
The CNPM will partner with community physicians to address the functional, clinical, and psychosocial needs of participants, emphasizing independence and wellness.
This role serves as a vital link between participants, community physicians, and plan staff to develop and implement comprehensive care plans, including person-centered goal setting, communication of changes in participant conditions, and facilitation of preventive primary care services.
JOB RESPONSIBILITIES:
- Each CNPM will be assigned to a specific site where they will oversee all participants.
- Supervises and manages Nurse Practitioners assigned to their location and provides oversight for other clinical staff as necessary.
- Conducts direct reports and annual performance evaluations, collaborating with other managers to provide feedback to clinical staff.
- In the absence of the Medical Director, the CNPM will assume the Medical Director's responsibilities at their location.
- Conducts annual wellness assessments for all assigned participants, ensuring an understanding of the complexities and comorbidities of the elderly and frail population.
For participants with higher risk levels, these assessments will be conducted more frequently to adequately address their needs.
- Provides urgent care services to participants and facilitates same-day access.
- Assesses and manages participants experiencing acute changes in their condition promptly.
- Evaluates participants returning from hospital or emergency department visits within five days, including medication reconciliation and coordination with the primary care physician.
- Reviews patients' medical histories biannually, creating a comprehensive diagnostic list of current and past conditions based on clinical findings.
- Ensures all documentation is accurate and complete, with specific diagnosis codes recorded in the clinical platform as required.
- Participates in clinical documentation improvement initiatives to accurately capture the burden of illness and ensure appropriate risk adjustment.
- Reviews participants' current symptoms and identifies active diagnoses for effective care management.
- Collaborates with primary care physicians to manage high-risk participants and communicates assessment findings to address potential care gaps.
- Attends interdisciplinary team meetings and represents community physicians when necessary.
- Communicates effectively with the interdisciplinary team and other designated staff to ensure comprehensive care delivery.
