Position Purpose: The supported housing team is recovery and resiliency oriented, intensive, and transitional in nature. It is team based and consists of mental health rehabilitation interventions supporting what is necessary to assist the recipient in achieving and maintaining resiliency and recovery goals. The supportive housing team is designed to meet the educational, vocational, residential, mental health, financial, social and other treatment needs of the recipient. Interventions are provided in natural settings, and are delivered face to face, by telephone, or by video conference with individual recipients and those involved in their care.

This position will serve as the team leader of the supported housing program. This program houses up to 38 clients in apartment settings spread between two buildings. The staff provide services in the community, in client apartments, and in the offices which are on site. Beyond supervision of the staff, the team leader will also be the point person for referral, and discharge planning. The team leader will work closely with other teams to identify appropriate clients for the setting as well as work closely with peripheral services. This position will serve in a support role when needed for coverage at two CILA sites. The team leader will participate in the on call rotation for the CILA sites. **(Work Hours: Monday - Friday 8:30am - 5:00pm)**

Qualifications/Skills:

Bachelor's Degree in Psychology, Social Work, or related field - Master's preferred. Must qualify as a MHP.

The ability to supervise, communicate clearly verbally, write concisely and legibly.

Counsel, edit, estimate, evaluate, influence, measure, observe, organize, plan priorities, summarize, and continual use of good judgment.

Ability to quickly establish rapport with others

Ability to perform in an ethical manner.

Ability to follow written and oral instruction, and write effectively.

Ability to work as a team member, and provide supervision to members of the team.

Valid drivers license

Essential Responsibilities:

1. Work hours prescribed and any additional hours deemed necessary for the responsibilities assigned.

2. Provide oversight and direction to the supported housing team.

3. Provide clinical supervision to team members.

4. Complete requirements for review and signing of all required clinical documentation.

5. Provide support to CILA sites as needed, and participate in the on call rotation.

6. Ensure that individual and group census numbers are within required fidelities.

7. Provide individual, group and family services (55 hours per month).

8. Provide crisis intervention services as needed.

9. Understand and comply with all of the principles established by the Rosecrance Corporate Compliance Program and Code of Conduct.

10. Perform all responsibilities in compliance with the mission, vision, values and expectations of Rosecrance.

11. Serve as a role model for other staff, clients and customers and demonstrate positive guest relations in representing Rosecrance.

13. Assume other related responsibilities as assigned.
