Rapid Rehousing Specialist

Position Summary: To provide rapid rehousing (RRH) advocacy and case management services to survivors/victims of domestic violence and/or human trafficking. Case management, using a housing first approach, will include trauma-informed, survivor-centered methods to housing stabilization, rebuilding self-esteem, developing essential life skills, and establishing financial independence by supporting clients' goals to increase income and self-sufficiency.

Accountable and responsible for the following results:
  • Complete coordinated intakes, assessments, and screenings to determine housing eligibility based on HUD and Connecticut State Department of Housing (DOH) guidelines.
  • Complete survivor-centered, strengths-based housing plans that identify barriers to housing and set a path to housing stability; reviewed and updated monthly.
  • Support in designing and implementing safety plans and housing risk assessments
  • Conduct visits in the home/community, at a minimum of once per month, with each survivor enrolled in the program. Follow organizational protocols for at home visits.
  • Provide case management services focused on increasing income, access long-term housing subsidies, establish and/or expand family and community-based supports, and assist in increasing access to mainstream services.
  • Foster relationships within the homeless service system.
  • Provide regular update to department staff on status of RRH availability.
  • Assist in program development and quality assurance activities as directed by CCADV Director of Housing Advocacy.
  • Assist survivors with locating available housing units, the leasing process, landlord mediation, and tenant/landlord rights and responsibilities.
  • Work with survivors to increase income, ensure health insurance coverage, and support efforts to secure employment.
  • Submit requests for rental payments for survivors enrolled in the RRH program.
  • Other duties as assigned.
DOCUMENTATION
  • Maintain accurate and up to date files and records to include required documentation and confidential case management notes.
  • Submit timesheets and personnel activity reports (PARs) monthly to CCADV
  • Submit annual report to CCADV 30 days after the grant end date.
  • Document all client and program data into ETO (Efforts to Outcomes) system within 48 hours of contact.
  • Compile data for required reports (i.e. ETO, RRH HMIS) quarterly and send to CCADV
  • Complete program narrative for reports per funder requirements.
  • Required to use own vehicle on company business or to transport clients
  • Regular and consistent attendance is an essential function of this position

TRAINING & PROFESSIONAL DEVELOPMENT
  • Attend meetings (CAN, etc.)
  • Participate in the annual Point-in-Time count
  • Attend monthly RRH TA sessions
  • Participate in the RRH Case Conference team meetings
  • Attend CCADV trainings and workshops related to housing
  • Maintain DV certification by receiving not less than six hours of in-service training in issues related to working with survivors of domestic violence / human trafficking
  • May perform other assignments and duties as assigned

Qualifications

Must be able to attend required training during first week of employment, which is Monday through Friday, 8:15 a.m. to 4:30 p.m.
  • Bachelor's Degree required;
  • Two years Advocacy and/or direct service experience working with survivors of domestic violence in a community-based setting preferred;
  • Bi-lingual (English/Spanish) reading writing and speaking preferred; Effective oral and written communication skills.
  • Ability to work effectively with a diverse range of clients to provide crisis intervention, case management, resource and referral supports. Knowledge of community resources
  • Comprehensive working knowledge of safety planning and survivor-centered advocacy as it relates to domestic violence survivors
  • Ability to maintain service records, complete records accurately and submit in a timely fashion in accordance with CCADV Program Standards.
  • Willingness to learn how to navigate multiple systems including housing, health, employment, substance use disorder treatment and government benefits.
  • Demonstrated ability to work collaboratively with others, within the DV system and with outside providers, to develop creative solutions to difficult challenges.
  • Must be proficient in Microsoft 365. Must be able to navigate through electronic health records system (EHR).
  • Must have and maintain a valid driver's license. Must maintain an acceptable driving record and be insurable at a reasonable rate under the Organization's auto insurance plan.
LICENSURE /CERTIFICATIONS:
  • Must be able to obtain and maintain CPR/ First Aid certification.
  • Must be able to obtain and maintain the certified domestic violence counselor per CGS 52-146k before providing direct service to clients, followed with six-hour recertification yearly.
