Housing Coordinator

Company: Los Angeles Christian Health Centers (LACHC)

The Housing Coordinator position plays a crucial role in supporting LACHC's mission of providing compassionate healthcare services to individuals experiencing homelessness. This role involves working closely with patients to assess housing needs, develop comprehensive housing plans, and secure permanent housing solutions.

ESSENTIAL RESPONSIBILITIES:

  • Engage with patients through in-person outreach strategies
  • Utilize Housing First approach and knowledge of Fair Housing rights
  • Access nonprofit and community resources for homeless individuals
  • Implement Trauma-Informed Care and Motivational Interviewing techniques
  • Utilize evidence-based interventions for effective housing navigation
  • Maintain accurate records and billing in the Electronic Health Record system
  • Provide support for various populations, including those with mental health and substance use disorders

Housing Navigation Responsibilities:

  • Discuss housing options with patients and assist in navigating housing processes
  • Enter data accurately into the Homeless Management Information System
  • Assist patients in applying for housing vouchers
  • Collaborate with Lead Care Managers to monitor progress towards stable housing
  • Provide assistance with necessary documentation for housing applications

Tenancy Support Responsibilities:

  • Educate patients on budgeting for housing maintenance
  • Conduct in-home visits to ensure patient well-being
  • Support patients in property management interactions
  • Address Fair Housing Regulations with patients and landlords
  • Ensure proper documentation of services provided

QUALIFICATIONS:

  • CPR Certification required
  • Bi-lingual Spanish proficiency preferred

EDUCATION and EXPERIENCE:

Minimum of two years of case management experience required. Bachelor's degree in social work or related field preferred. Past experience with homeless populations and mental health conditions is advantageous.

COMPUTER SKILLS:

Proficiency in Electronic Health Records, Microsoft Office, and database management systems is preferred.

LANGUAGE SKILLS:

Familiarity with medical terminology is beneficial. Fluency in English required, Spanish proficiency preferred.

REASONING ABILITY:

Ability to solve practical problems and make informed decisions. Capacity to interpret various instructions and work effectively in high-pressure situations.

WORK ENVIRONMENT:

The role may involve exposure to contagious diseases and challenging client interactions. Maintaining patient confidentiality is paramount.
