As a key member of our Avivo Village team, you will play a crucial role in supporting our residents and ensuring they receive the highest quality care and services. Your work will directly impact the well-being and success of those we serve, fostering an environment of healing and growth.
Key Responsibilities Include:
Providing compassionate, trauma-informed care to residents. Collaborating with team members to develop and implement individualized care plans. Engaging with residents to build trust and promote their well-being. Facilitating access to resources and services that support residents' recovery and independence. Maintaining accurate and timely documentation of interactions and services provided. Participating in team meetings and contributing to continuous improvement efforts.
