Job Summary:
Patient Navigator of Prostate Cancer will oversee that prostate cancer patients return to the clinical practice per an established care pathway.
Responsibilities and Duties:
- Use EMR and PPS software to keep track of where patients are in the care pathway
- Assist, in conjunction with the Providers, in establishing an effective Patient and Clinical Workflow
- Ability to establish effective, trusting working relationships with patients
- Follow the Clinical Care Pathway to contact patients at designated times in their journey
- Ensure patients are aware of care instructions and commit to follow up appt (and what to expect in the appt).
- Answer any questions the patient may have (may require follow up call).
- Exercise problemsolving and analytical skills to find create solutions to patient barriers
- Maintain accurate and complete documentation in the patient records and PPS Software
- Bring forward to Manager any issues with the process / care pathway as soon as they occur
- Understand HIPAA and importance of privacy of patient data
- Location will be once a week at the Radiation Center, then base office remaining of the week
- All other duties as assigned.
Qualifications and Skills:
- Experienced MA or LVN
- Knowledge of prostate cancer disease states and therapies
