As a Social Worker at Interim Healthcare, you play a pivotal role in understanding and addressing the psychosocial needs of our patients. By taking the time to truly comprehend each patient's background, we can create a safe and comfortable environment for them within their own homes.

General Purpose: Responsible for enhancing the overall quality of care, focusing on meeting the psychosocial requirements of the patient/client in alignment with professional standards and company policies.

Essential Functions:

  • Assess and address the psychosocial needs of the patient/client per physician's care plan.
  • Report on patient/client condition to supervisors and team members.
  • Provide social work services including counseling and crisis intervention.
  • Collaborate with the healthcare team to understand emotional needs related to patient's health issues.
  • Develop care plans in consultation with physicians and team members.
  • Record treatment details and patient progress accurately and promptly.
  • Coordinate care for patients/clients and suggest appropriate services.
  • Utilize community resources as necessary.
  • Communicate effectively with the healthcare team.
  • Participate in on-call duties and quality improvement measures.

Minimum Education & Experience Requirements:

  • Master of Social Work (MSW) degree preferred.
  • State license to practice as applicable.
  • Minimum of 12 months experience in the last 5 years.

Skills, Ability & Knowledge:

  • Excellent communication and multitasking skills.
  • Ability to interpret technical instructions.
  • Observational and assessment skills.
  • Ability to travel locally.
  • Valid driver's license and auto insurance.
  • Compliance with health requirements.
  • Proof of Covid vaccine or exemptions.
