The Social Worker II (SWII) performs advanced social service work within a social learning matrix that requires analytical skills, consistent clinical interpretation and sound judgment based on standard principles of Social Work. Serves patients with mental, emotional and behavioral conditions and addictions. Serves as a liaison between patients and their families in obtaining assistance and rehabilitative treatment, and identifies patient needs through psychosocial assessments; treatment team collaboration, duties and meetings. Provides continuity of care functions and discharge planning with the patient, families, LMHA’s and other agencies, as appropriate. Demonstrates an appropriate level of clinical expertise in discussion of case dynamics and behavior of patient that contributes to the treatment team’s knowledge and understanding of the patient to provide the highest quality of care. The SW II reviews and finalizes discharge documents for LBSW/Case Managers. Supervises assigned social work student intern and provides clinical training to new staff. The SW II works under minimal supervision with extensive latitude for the use of initiative and independent judgment; and functions under the supervision of the Director of Social Services. Essential Job Functions:
Attends work on a regular basis and may be required to work a specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime in accordance with agency leave policy and performs other duties as assigned.
1. Completes Social Assessments, prepares Social History Updates, and identifies educational needs of patient and family.
2. Provides discharge planning and continuity of care to patient through contacts with the patient’s family (if consented by patient), mental health authorities, and other providers.
3. Actively participates as a member of the Interdisciplinary Treatment Team to promote a unified, effective team approach using the expertise of all team members to assure high quality patient care and to provide social work assessment information, family and Continuity of Care's (COC) input into the patient’s recovery planning process.
4. Fulfills clinical documentation requirements to accurately evaluate the patient’s progress in recovery, ongoing patient and family educational needs and discharge/aftercare needs.
5. Assures that family contacts are established and maintained for each patient (giving consent) in a courteous/professional manner to ensure continuity of care and provide family education.
6. Finalizes discharge documents in the electronic record-keeping system for non-licensed staff.
7. Serves as a mentor to less experienced social workers to help patients receive the highest possible level of social work services.
8. Attends assigned committee meetings to participate in achieving the goals of the facility.
9. May supervise the work of others.
10. Performs related work as assigned.
Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency’s obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Such participation may require an alternate shift pattern assignment and/or location. Knowledge Skills Abilities:
