The Counselor provides counseling services and coordination of care for patients and families, utilizing individual, group, and/or family approaches as indicated by need.

Maintains and manages a caseload of patients that varies in size depending on need.

Responsible for service delivery including conducting intake assessments, participating in Child and Family Teams, and providing clinical oversight to teams.

Works with patients and their families to create individualized service plans, implements agreed upon interventions and monitors progress toward goal completion in accordance with the Arizona Vision and 12 Principles.

Works team to prevent and/or appropriately respond to crises and to minimize out-of-home placement. Monitors patients' level of acuity/need and recommends appropriate level of case management and/or psychiatric services accordingly. Promotes patients/family independence and assists with transition planning.

NHSC Loan Repayment Program - awards up to $50,000 in exchange for a two-year commitment** to provide primary medical, dental, or mental/behavioral health care at approved sites in high-need, underserved areas. Qualifications

  • 18 years of age or older; and
  • Master's degree in a counseling related field is required (professional counseling, marriage and family therapy, social work, counseling psychology, substance abuse counseling)
  • Preferred but not required upon hire: Hold an active associate level or independent level behavioral health license, as approved by AZBBHE (LMSW, LAC, LAMFT, LCSW, LPC, LMFT)
  • Applicant can start within 90 days of graduation, continued employment will be contingent upon verification of degree
  • Must have the skills and knowledge necessary to provide the behavioral health services that the agency is authorized to provide and to meet the unique needs of client populations served by the agency, including children, and meet requirements for a BHT II or BHP.
  • Exceptional written and verbal communication skills and must be able to execute conflict resolution.
  • Ability to develop routine reports and correspondence.
  • Knowledge of Windows 365 environment and/or Electronic Health Records Systems.
  • Ability to provide culturally sensitive services
  • Must have or be able to obtain a valid Fingerprint Clearance Card.
  • Must possess reliable transportation, valid driver's license, and proof of auto insurance.
  • Must maintain current BLS certification
  • Covid 19 Vaccination or Qualified Exemption (Religious or Medical).
  • TB Screening Record and PPD/QuantiFERON/Chest xray as applicable upon hire
  • Vaccinations/Titer's Test/
