GENERAL FUNCTION
The Community Health Worker (CHW) serves as an integral member of the Healthy Living Team.

The Community Health Workers' primary responsibility will be focused on assisting individuals within the YMCA by providing support and education surrounding overall health.

The Community Health Worker will work directly with the individual, families, and community partners with the intention of improving health outcomes for individuals served by the YMCA of Greater Indianapolis.


Under the direction and supervision of the Community Health Specialist, the Community Health Worker will be primarily responsible for delivering the Diabetes Prevention Program in identified priority populations.

The Community Health Worker will also assist with the recruitment and enrollment of participants into the other YMCA's Chronic Disease Preveniton Programs; including but not limited to the YMCA's Diabetes Prevention Program, LIVESTRONG at the YMCA, Blood Pressure Self-Monitoring Program, Enhance Fitness and Moving for Better Balance at the Y, by helping participants achieve their physical health and well-being goals.


KNOW HOW
A high school diploma, or its equivalent is required.

Must have existing Community Health Worker certification or agree to complete Community Health Worker training provided by the YMCA and achieve certification within first 6 months of hire.

Must have exceptional communication and organizational skills in order to support participants and maintain required documentation for program. 1 year of experience leading/facilitating classes and working with special populations is preferred. Knowledge of basic health and wellness principles and exercises, as well as healthy living behaviors is preferred. Knowledge and experience with health behavior and challenges associated with making lifestyle changes preferred. Verbally provide clear concise directions and visually inspect classroom for safety. Ability to maintain confidentiality of program participants.

Must be willing to complete YMCA provided trainings and certifications in HIPAA, CPR AED, First Aid, Safety & Risk Management, Child Abuse Prevention and New Employee Orientation the ability to obtain within 60 days of employment.


PRINCIPLE RESPONSIBILITIES

  • Assist with strategizing and implementing programs to reduce barriers to access and improved overall health to the population served.
  • Assist with identifying an addressing individual and community needs around health education and access to resources.
  • Assist with interpreting data from assessments to identify appropriate next steps for program sustainability in identified communities.
  • Provides community resource referrals, participant advocacy, support, outreach, and followup.
  • Provide instruction to class participants in effective and meaningful ways.
  • Introduce yourself and make efforts to recognize members on subsequent visits.
  • Develop and maintain relationships that promote the building of small communities.
  • Create a motivating environment that is friendly and noncompetitive.
  • Be available to class participants both before and after classes to answer questions and followup on any questions you cannot answer during class time.
  • Maintain Continuing Education Credits and other trainings to keep all certifications current.
  • Conduct self in accordance with the YMCA Core Values; Caring, Honesty, Respect, Responsibility.
  • Actively participate in the annual campaign.
  • Attend all staff meetings and training associated with Chronic Disease Programs at the YMCA.
  • Enter data into appropriate data system immediately after class, and securely access/utilize/store data in accordance with privacy practices
  • Available to lead Chronic Disease Prevention groups in the evening and at multiple sites/locations.
  • Assist with other projects or activities as needed.
