ESSENTIAL DUTIES AND RESPONSIBILITIES:

Provide screening, assessment, individual, family, and collateral therapy services with clients referred to the PIER Program who are exhibiting early high-risk clinical signs of psychosis
Also will provide Multifamily Groups as part of the PIER EBP model of treatment.


Consult and coordinate with PIER treatment team, including the Nurse Practitioners and/or Psychiatrists, in case planning, case disposition, linkage to other needed community services and resources, and in discharge planning.

Responsible for meeting the required Productivity expectation on an ongoing and consistent basis.

Responsible for Chart Documentation including assessments, log sheets, progress notes, and all additional documentation as required.


Ensure chart documentation is completed and submitted within required timelines; specifically, Progress Notes are completed utilizing the Collaborative Documentation model during sessions and then submitted after sessions or within 48 hours.

Provide Case Management and Linkage in coordination with the treatment team to assist with overall family functioning.
