Person-centered Focus:
Community Integration Specialists (CIS's) carry out a wide range of personalized supports and services with a strong emphasis on individual choice for individuals with developmental disabilities. These services involve advocating for, encouraging, guiding, and teaching individuals in expressing personal choice, developing daily living skills, and maintaining a healthy and safe home environment.

Key Competencies Required:

Position Summary:
  • Offer necessary support for individuals to achieve their desired lifestyles and enhance independence and community inclusion.
  • Tasks include developing and assisting with person-centered planning, transportation, supervision of daily activities, and documentation.

Qualifications:
  • High school diploma or equivalent and 18+ years of age.
  • Computer literate with strong oral and written skills.
  • Dependable, reliable, and possesses strong communication skills.
  • Ability to problem-solve, make decisions, and maintain certifications.
  • Excellent interpersonal skills and customer service.
  • Valid driver's license and reliable transportation.

Essential Functions:
Preparation for Change:
  • Plan and prepare individuals for upcoming changes.
  • Introduce individuals to new activities based on their interests.
  • Teach necessary skills for success and provide support.
  • Review and update individual plans.

Community Integration and Participation:
  • Assist in planning and implementing community experiences.
  • Support goals related to community involvement and employment skills.
  • Accompany individuals to community activities and promote inclusion.
  • Teach community living skills and positive work habits.
  • Model behavior for individuals with and without disabilities.

Personal and Health Care:
  • Assist with daily living activities.
  • Ensure individuals' well-being and safety.
  • Maintain a safe environment and administer medication.
  • Handle emergencies and provide first aid.

Communication and Documentation:
  • Complete required documentation accurately.
  • Participate in staff meetings and training sessions.
  • Support organizational policies and missions.
  • Demonstrate teamwork and effective communication.
  • Participate in individual goal planning and community experiences.

Managerial Duties:
  • Provide support and communicate effectively with individuals.
  • Instruct in work-related skills and act as a community liaison.
  • Ensure professional communication and respect individuals' rights.

General:
  • Track attendance and submit required reports.
  • Complete regulatory reports and maintain proper documentation.


Supervision:
  • Provide supervision to individuals receiving support.

Mental, Physical, and Visual Demands:
This role involves extensive computer use and physical tasks as needed to support individuals.
Working Conditions:
Requires travel throughout the county and the community, may involve standing, lifting, and working in various conditions.
