Overview:


The registered nurse plays a crucial role in planning, organizing, and overseeing hospice care, with a strong background in nursing and community health education/experience.

Responsibilities:
- Conduct initial, comprehensive assessments to identify hospice care needs of patients and their families
- Perform thorough physical assessments and review medical histories
- Deliver professional nursing care by applying the nursing process effectively

Evaluating patient's status involves:
- Developing and initiating individualized care plans
- Regularly reassessing patient and family/caregiver needs
- Collaborating in adjusting care plans when necessary
- Taking proactive steps to address changes in patient conditions and needs
- Using health assessment data to formulate nursing diagnoses
- Creating care plans with specific goals based on nursing diagnoses and integrating palliative nursing measures
- Involving patients and families in the planning process
- Implementing appropriate preventive and rehabilitative nursing interventions
- Administering medications and treatments as prescribed in the physician's care plan
- Providing counseling to patients and families on nursing requirements
- Offering health care instructions based on assessments and care plans
- Assisting patients with daily activities and encouraging self-sufficiency and comfort care
- Acting as a Case Manager to coordinate patient care within assigned caseloads

- Documenting accurate and pertinent clinical notes on patient conditions and care provided
- Recording changes/outcomes in pain/symptom management as necessary
- Liaising with physicians regarding patient needs and reporting condition changes; obtaining/receiving necessary orders
- Coordinating care plans with community health professionals
- Educating patients and families/caregivers on self-care techniques
- Providing medication, diet, and other instructions as per physician orders
- Identifying and utilizing opportunities for health counseling with patients and families/caregivers
- Collaborating with the interdisciplinary team
- Ensuring a safe environment for patients
- Promoting care continuity among patients, families, caregivers, and team members
- Working closely with families/caregivers and the hospice team to address emotional needs
- Participating in interdisciplinary meetings
- Handling on-call duties as per policy guidelines
- Securing necessary equipment and services for patient care
- Supervising support staff and delegating tasks as needed
- Continuing personal and professional development through education and training sessions
- Fulfilling assigned case responsibilities efficiently
- Actively engaging in quality assessment and performance improvement activities

Qualifications:
- Graduate of an accredited nursing school
- Valid Registered Nurse license in the State and current American Heart Association CPR certification (can be obtained upon hire)
- Preferred Bachelor's degree; one (1) year of hospice experience is advantageous
- Prior management experience is not mandatory; however, oversight of hospice aides is part of the role
- Strong observational, communication (verbal and written), problem-solving, and basic math skills; nursing competency per checklist
