Job Description

POSITION SUMMARY:

Provide clinical services in an hospital-at-home pilot including acute and longitudinal care in a home setting, in collaboration with attending physician.


ESSENTIAL FUNCTIONS:
Qualified individuals must have the ability (with or without reasonable accommodation) to perform the following duties:

  • Lead and develop the medical and programmatic components of Penn Medicine Advanced Home Health.
  • Coordinate and supervise the APP staff
  • Consult with patients to determine eligibility for Penn Medicine Advanced Home Health program and establish the appropriate course of treatment.
  • Recommend and order tests to determine extent of illness or help diagnose condition.
  • Monitor lab results, medication adherence and medication toxicities.
  • Prescribe or administer treatment, therapy, medication, vaccinations, and other medical care to treat or prevent illness, disease, or injury.
  • Explain and discuss procedures, test results, or prescribed treatment with the patient.
  • Provide in-person home follow up visits or telehealth visits to assess needs
  • Provide phone coverage of clinical questions from home health team or APP during weekday hours
  • Coordinate work with PCP and the home health team including nurses, social workers, rehabilitation therapists, pharmacists, psychologists, and other health care providers.

SECONDARY FUNCTIONS:
The following duties are considered secondary to the primary duties listed above:

  • Provide medical care plan consultation during meetings or phone calls
  • Provide clinical and didactic teaching to staff on related topics and issues.
  • Complete other duties as assigned.
  • Participate in the on-call responsibilities for this position as required.

JOB REQUIREMENTS

MINIMUM REQUIRED QUALIFICATIONS:

  • A Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.)
  • Completion of Internal Medicine Residency or Family Medicine Residency.
  • Board Eligible or Board Certified in Internal/Family Medicine.
  • Current medical licensure issued by PA State Board of Medicine or the PA State Board of Osteopathic Medicine.
  • A health care provider in good standing with Medicare, Medicaid, and other federal and state health insurance programs, i.e. not excluded from participation in Medicare, Medicaid or any other federal or state health insurance program.

PREFERRED QUALIFICATIONS:

  • At least three years clinical patient care experience as a general internal or family physician with acute care and/or hospitalist experience.
