Palm Medical Centers is a company on the rise and is currently in search of a skilled RN Clinical Coordinator with leadership expertise to join the team.

The RN Clinical Coordinator handles various tasks and oversees the care plans for hospital and nursing home patients, ensuring effective and efficient care from admission to discharge.

Directly reports to the Senior Director of Case Management in the Utilization Management Department.

Responsibilities include:

  • Admit new clients after reviewing records and conducting orientations
  • Track daily hospital/nursing home admissions and discharges for statistical and cost management
  • Determine clients' requirements through intake interviews; evaluate treatments and responses; advocate for services
  • Establish treatment programs, set schedules, and routines; monitor cases and attendance
  • Maintain clients' records, communicate with managers on member discharges
  • Coordinate patient transfers, arrange admissions and placements
  • Assist in monitoring patients' length of stay and facilitate necessary discharge arrangements
  • Conduct weekly interdisciplinary meetings and evaluations to report clients' progress
  • Enhance staff competence and treatment results through educational resources and process improvements

Requirements:

  • Proven experience in case management, including RN, medical or mental health care management
  • Knowledge of case management principles, communication, and organizational skills
  • Experience with psychological care aspects, problem-solving, and multitasking abilities
  • Compassionate, team player with familiarity in CPT codes and ICD 10
  • Active TX RN license and a minimum of 3 years as an RN Clinical Coordinator

Job Type: Full-time
Salary: From $70,000.00 per year (negotiable based on experience)
