Position Overview

As a (PASRR) Preadmission Screening and Resident Review Clinical Evaluator, you will conduct onsite assessments to facilitate appropriate placement and care for local populations in Bremerton, Washington. Your role involves providing essential evaluations and recommendations to enhance patient well-being and quality of life. These assessments, primarily conducted in nursing homes or hospital settings, play a crucial role in determining the level and scope of care required for each patient. Your assessments will be instrumental in securing better health outcomes, providing critical support that aligns with the specific needs of each individual.

As a trusted expert, you will have the autonomy to evaluate their care needs and determinations and recommendations for their ongoing level of care, helping to improve patient's well-being and enhance their quality of life. This is a PRN hybrid role that combines working from your home office with traveling to patients within your local areas. Must be open to covering the Bremerton area. All required travel expenses, including mileage will be fully reimbursed.

Job Responsibilities
  • Conduct screenings, evaluations, and determinations by thoroughly assessing patients, relevant records, and collateral sources of information.
  • Determine the appropriate level of care and service needs based on the patient's physical, behavioral, and psychological functioning.
  • Apply knowledge of psychiatric disorders as defined by the most current Diagnostic and Statistical Manual used in Washington.
  • Adhere to standards of practice related to mental health, intellectual and developmental disabilities, geriatrics, and other conditions necessitating long-term care services and supports.
  • Demonstrate proficiency in Microsoft Excel, Word, and Outlook for efficient documentation and communication.
  • Confirm clinical diagnoses, accounting for psychological and behavioral factors.
  • Utilize computer equipment and web-based software to facilitate work processes effectively.
  • Interact professionally with healthcare professionals, patients, and their families as necessary.
  • Communicate effectively to individuals and groups through various mediums, including spoken, written, and electronic forms.
  • Monitor personal metrics to ensure compliance with turnaround time requirements.
  • Ensure compliance with necessary workflows to maintain quality and efficiency.
  • Attend and actively participate in meetings and conduct case consultations/peer reviews as needed.
  • Stay updated on current regulations, and research related to long-term care, mental illness, medical conditions, and geriatrics.
  • The list of accountabilities is not intended to be all-inclusive and may be expanded to include other duties that management may deem necessary.
Required Qualifications, Skills, Knowledge and Experience
  • Minimum 3 years of experience in assessing and diagnosing individuals with mental/psychiatric disorders, including complex differential diagnoses related to substance use, medical conditions, cognitive issues, and dementia.
  • Active unrestricted licensure in Washington in a healthcare or related field, such as a Licensed Psychologist, Psychiatric Nurse, Social Worker, Mental Health Counselor, Marriage and Family Therapist, or Social Worker Independent Clinical License.
  • Graduation from an accredited college with a relevant degree program.
  • Working knowledge of Preadmission Screening and Resident Review (PASRR) requirements per Federal Regulation 42 CFR, Chapter 483.
  • Familiarity with psychiatric disorders as defined by the most current Diagnostic and Statistical Manual used in Washington.
  • Skill in developing level-of-care recommendations based on clinical evaluations.
  • Knowledge of and/or experience with public mental health and medical programs.
  • Understanding of standards of practice related to determining the Level of Care.
  • Knowledge of standards of practice related to mental health, geriatrics, and other disorders commonly necessitating long-term care placement.
  • Experience working with the geriatric population.
  • Strong understanding of public sector services and supports.
  • Proficiency with Microsoft Office applications, including Excel, Word, PowerPoint, Teams, and Outlook.
  • Ability to effectively use computer equipment and web-based software for conducting work.
  • Capability to collaborate with various office staff to support essential workflows.
  • Proficient in interacting with healthcare professionals, patients, their families, and other support systems.
  • Excellent communication skills for conveying information to individuals and groups through spoken, written, and electronic media.
  • Keen attention to detail, with the ability to prioritize and execute tasks efficiently and in a timely manner.
  • Expertise in assessing and analyzing psychological, emotional, behavioral, and physical functioning.

Compensation

The compensation for this role is $40.00 to $44.00 per hour. Based on our compensation program, an applicant's position placement in the pay range will depend on various considerations, such as years of applicable experience and skill level.
