Work collaboratively with participants, their family members, DARS counselors, school personnel, other partners, employers, business and education representatives to develop and refine relevant employment transition opportunities as well as cultivate partnerships with employers and educational programs.

Primary DutiesReasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. These duties and responsibilities will be rated on the Annual Performance Review.

Conduct engaging, relevant, informative, and dynamic group sessions with high school students with disabilities, leading activities that cover the range of Pre-Employment Transition Services.

Lead outreach and recruitment efforts for attendees for new groups throughout the year, including disseminating outreach flyers; holding information sessions; and answering questions as needed from referring partners, interested participants, and family members.

Develop and maintain positive relationships with partner agencies, school personnel, businesses, educational institutions, other departments and/or vendors to enhance menu of services, continuum of care, and the number of employment transition opportunities.

Provide general case management, maintaining excellent communication with participants, family members, funding and referring agencies, schools and other partners; sending monthly requests for authorizations.

Includes communicating with funding and referring agencies regarding the direction of services; attending and leading all Pre-ETS group meetings as scheduled; scheduling one-on-one appointments with participants as needed; and attending team meetings.

Complete in an accurate and timely manner all monthly billing reports, participant reports, case notes, and other supporting documentation, meeting utilization expectations.

Additional Responsibilities Develop group-based curriculum aligned with students needs following the Pre-ETS guidelines and expectations, both in-person and virtually.
Manage own schedule and accurately document time spent with each individual.

Be aware of and respond accordingly to any specific needs of the participants in the groups, including mental health safety, accommodations needed in a group setting, etc.

Conduct tours and informational interviews with representatives from employers and educational programs to choose the most relevant and highest quality settings for participation in pre-employment transition group activities.

Provide consultation, guidance, and disability awareness training (as needed) to employers and educational program representatives to foster positive partnerships.

Attend relevant funding and referring partner meetings to discuss participants, receive feedback and updates, and market services with ServiceSource to increase number of referralsCoordinate and participate in support team process and other meetings pertaining to participant's individual support plan.

Prepare annual individual support plans as neededPerform other related duties as assigned.

Qualifications:
Education, Experience, and Certification(s) Bachelor's degree in human services, education, or marketing field required. One (1) year of education can be substituted for one year of related experience.
One (1) year experience working with transitioning students, young adults with disabilities, or other relevant, transferrable work experience, required.
Valid driver's license and/or access to reliable transportation to perform work-related travel required.

Eligible drivers must have a good motor vehicle record (MVR).Transport participants on tours of employment and educational settingsAbility to travel weekly to multiple locations in the local area, at times with little notice required.

Availability and willingness to work flexible hours, including evenings and/or weekends, when neededExperience public speaking, training, or teaching in a group setting preferred.

Experience developing partnerships with employers and/or postsecondary educational institutions preferred.
Fluency in a second language (ASL or Spanish) preferred.

Knowledge, Skills, and Abilities An individual who can relate well to people at all levels of the organization, as well as provide broad support to high school students with a variety of disabilities.

Excellent public speaking skills, with the ability to follow a curriculum, customize activities as needed, and train and teach in a group setting in an engaging and informative manner.

Detail-oriented and able to carry out work with the highest levels of accuracy.
Goal oriented and independently motivated.
Ability to work independently and as part of a team.
Excellent customer service skills (virtual and face-to-face).Ability to solve practical problems and adapt to new information and guidance quickly.
Excellent oral and written skills, as well as ability to respond effectively and respectfully to sensitive inquiries or complaints.

Advanced Proficiency in Microsoft Office skills, and the ability to become familiar with corporate specific programs and software such as ZOOM and Microsoft Teams.
