ResponsibilitiesInvestigate, assess and plan interventions to help patients cope with social, emotional,economic and environmental problems+ Interview patients, assess priorities and document case activity.+ Consult with team managers, statutory and voluntary agencies and patient's relatives to ensure compliance with patient's treatment plan.+ Assist clients/families with obtaining community assistance by referral to proper resources.+ Develop and maintain working relationships with community health, welfare and social agencies and seeks creative means to assist the client with his/her needs.+ Support management of concurrent review, discharge planning processes, social services, home care and other post discharge needs.


RequirementsLicensure / Certification / Registration:

  • Drivers License preferred. Social Worker preferred. Licensed Social Worker credentialed from the Illinois Department of Financial and Professional Regulation, preferred.

Education:

  • Bachelor's degree in Social Work required. Master's degree in social work, psychology, or a related field required.

Work Experience:

  • 3 years of experience preferred. Previous experience in family therapy required and working in an inpatient psychiatric unit preferred.
    Additional PreferencesHybrid position-must live within 2 hours travel time of Des Plaines, IL*Must have LSW or LCSW*Must have Social Work Behavorial Health experience
