**Descrição Geral**: Sob a supervisão direta do Gerente de Programas da Prairieland Service Coordination, Inc., o Gerente de Casos avalia as necessidades individuais e garante a prestação dos serviços necessários por meio de comunicação, coordenação do serviço, ligação com agências prestadoras de serviços, monitoramento de serviços e defesa individual.

**Responsabilidades do Trabalho**:
- Realiza o processo PAS, seguindo os procedimentos do manual PAS.
- Advocacy em nome do indivíduo e/ou família para ajudar o indivíduo a obter serviços aos quais é elegível e que tenha escolhido.
- Garante o envolvimento direto e a participação do indivíduo e de sua família neste processo.
- Revisão das necessidades médicas, residenciais, vocacionais, educacionais, legais e financeiras do indivíduo e escolhas individuais através de Notas de Visita, conforme necessário.
- Agenda e conclui anualmente o processo de Descoberta e Plano Pessoal, ou conforme necessário.
- Mantém arquivos individuais conforme especificado pelas diretrizes do programa e da agência.
- Fornece consulta de resolução de problemas com agências fornecedoras seguindo a diretriz de Resolução de Conflitos da DHS.
- Coordena serviços especializados para indivíduos Bogard elegíveis conforme necessário e envia relatórios de resumo para agências externas, conforme apropriado.
- Monitora os serviços prestados aos membros da classe Bogard mensalmente e defende quaisquer necessidades não atendidas.
- Registra faturas diariamente no Birdseye para garantir que todos os relatórios de tempo para folha de pagamento sejam enviados pontualmente e com precisão.
- Participa de todas as reuniões agendadas de toda a equipe e sessões de treinamento obrigatórias conforme recomendado pelo Diretor Executivo e/ou Gerente de Programas.
- Alerta o Gerente de Programas do PSCI sobre questões problemáticas, tendências ou lacunas nos serviços.
- Realiza outros trabalhos relacionados conforme necessário ou solicitado pelo Diretor Executivo e/ou Gerente de Programas.

**Requisitos de Trabalho**:
- Diploma de bacharel em serviços humanos e elegibilidade para QIDP conforme estabelecido pelo Departamento de Serviços Humanos.
- Capacidade de se organizar, planejar e seguir um cronograma diário, semanal, mensal e anual.
- Conhecimento prático do sistema de prestação de serviços para pessoas com deficiências de desenvolvimento, incluindo requisitos e procedimentos de elegibilidade de destinatários.
- Habilidade de trabalhar e interagir efetivamente com profissionais, paraprofissionais, organizações de serviços sociais e clientes, incluindo suas famílias.
- Disponibilidade para seguir as diretrizes éticas dos funcionários, apresentando-se de maneira profissional por meio de comunicação oral e escrita diplomática.
- Meio de transporte confiável para o trabalho e manter uma Licença de Condução válida de Illinois e seguro auto pessoal com bom registro de veículos.
