We are seeking a Full-Time Physical Therapist for Home Health Visits.
Responsibilities- Evaluates the patient's functional status, status of all body systems as required for documentation, and physical therapy needs. Collaborates with the physician to develop the therapy plan.
- Provides services within the scope of practice as defined by state laws, in line with the plan of care, and in coordination with the healthcare team.
- Observes and reports the patient's response to treatment and any changes to the nurse supervisor and/or physician.
- Treats patients to relieve pain, develop and restore function, and maintain maximum functioning following standards of practice.
- Instructs and informs the health care team, the patient, and family/caregivers about the plan of care and progress toward goals.
- Visits patients within the service area and other nearby agencies as necessary.
- Completes progress notes for each visit and submits them according to policy.
- Completes all evaluations and develops the PT plan of care, reporting outcomes to the nurse supervisor.
- Participates in staff conferences and committees as requested.
- May supervise home health aides as allowed.
- Provides supervision of the PTA as per policy.
- Participates in a weekend rotation to provide services when needed.
- Prepares a written summary on the patient's condition every 60 days.
- Provides in-service education as requested.
- Adheres to Therapy Standards of Practice.
- Is responsible for ongoing education to stay current with standards of care and licensure.
- All other duties as assigned.
Licensure Requirements
- Current Physical Therapy licensure in FL
- Current CPR certification
- Must have a driver's license, vehicle insurance, and access to a dependable vehicle
