Job SummaryThe Physical Therapist will be responsible for providing physical therapy services to patients in their own homes. This includes assessing patients' conditions, developing personalized treatment plans, and delivering therapy to help improve mobility, reduce pain, and restore function.
Key Responsibilities- Conduct comprehensive assessments of patients' needs, including Outcome and Assessment Information Set (OASIS) assessments.
- Develop and implement treatment plans in consultation with physicians and revise as necessary.
- Assist in development and implementation of interdisciplinary client care plans as it pertains to Physical Therapy.
- Assist physicians in evaluating patients by applying diagnostic and prognostic muscle, nerve, joint, and functional ability tests.
- Treat patients to relieve pain, develop or restore functional and maintain maximum performance using physical means such as exercise, massage, heat, light, and electricity.
- Teach, supervise, and counsel patients and their families in the physical therapy program.
- Record all evaluation data, treatments, and patients' responses to therapy intervention in a timely manner.
- Prepare evaluations of patients' conditions and perform periodic reassessments.
- Supervise Physical Therapist Assistants at least every 30 days.
- Participate in case conferences, physical therapy staff meetings, agency activities, and in-service programs as necessary.
- Participate with staff, patients, and physicians in discharge planning activities and complete a Physical Therapy Discharge Summary.
- Evaluate patients, their significant others, and the home situation to determine what instruction will be required, what assistance will be available from the family in caring for the patient, and what other agency and community services will be required.
- A graduate of a Physical Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Physical Therapy Association.
- Licensed or registered by the State.
- At least one year of institutional experience.
- The Physical Therapist in charge of PT services must have a home care experience and at least one year of supervisory experience (2 years preferred).
- One year of experience in an acute/sub-acute setting.
