POSITION DESCRIPTIONPOSITION: Mattapan CARES Medical Case ManagerDEPARTMENT: Mattapan CARESLOCATION: 1575 Blue Hill Avenue, Mattapan, MA 02126REPORTS TO: Mattapan CARES Program ManagerTYPE: Full-TimeSALARY: NegotiableJOB SUMMARY: The Mattapan CARES Medical Case Manager provides medical case management to clients with HIV/AIDS, focusing on needs assessment, information and referrals, patient education, and advocacy.RESPONSIBILITIES90% Client Services:
  • Provides case management services to HIV patients.
  • Conducts assessments for HCV and PrEP patients, formulates care plans, revises plans based on patient needs, coordinates referrals, and reviews results.
  • Incorporates patient education into care plans, collaborates with healthcare team, makes home/hospital visits, and makes referrals to community agencies.
  • Attends team meetings, submits activity reports, and maintains clinical knowledge and skills.
  • Conducts intake and assessments on new HIV positive clients, reviews referrals, schedules appointments, and provides navigation services.
  • Tracks and follows up on PrEP and HCV referrals, conducts communications for adherence, tracks appointments and labs, and completes necessary forms.
10% Community Outreach and Health Promotion:
  • Participates in health center and community events to promote HIV, PrEP, and HCV services, engages new patients, and tracks testing performance.
Requirements:- Knowledge and experience with HIV services, substance abuse issues, and social service networks.- Experience working with diverse populations and basic computer skills.- Organizational, communication, attention to detail, and French Creole/Spanish language skills desirable.- Bachelor's degree preferred; High School/GED required.- Must comply with Corporate Compliance Program and perform assigned tasks.
