Job Title: Guardianship Representative Program Coordinator

Job Description

  • Implement, manage, plan, and coordinate the Guardianship Representative Program for Phoenix Counseling Center.
  • Ensure the needs of Phoenix Counseling Center's wards and their families are met through case monitoring, counseling, and decision-making.
  • Maintain regular contact with wards and families, including face-to-face meetings every 90 days.
  • Develop an annual care plan and progress reports for each ward.
  • Accept and manage assigned caseload diligently.
  • Build rapport with wards to enhance service delivery.
  • Adhere to administrative regulations, codes, laws, and statutes.
  • Advocate for the well-being of wards.
  • Participate in treatment team meetings and provide consultations.
  • Represent the Executive Director in various settings.
  • Adhere to Phoenix Counseling Center Employee Ethics Statement.
  • Perform other assigned duties.

General Knowledge and Skills

  • Comprehensive knowledge of guardianship laws and behavior management techniques.
  • Ability to work independently with discretion and meet deadlines.
  • Excellent judgment, initiative, and documentation skills.

Education/Experience

High school diploma or GED and 6 years of experience in MH/SA/IDD.

Physical Requirements

This position involves standing, walking, bending, lifting, and low physical exertion up to 80% of the time.

Additional Job Requirements

  • Effective communication skills and availability 24/7.
  • Reliable transportation and a valid driver's license with adequate insurance.
  • Ability to travel and operate office equipment.
  • Ability to write clearly and work in a high-stress environment.
