Job Summary:


The Admissions/Case Manager role is responsible for the timely response to admissions center inquiries and provides intensive case management services including continuing care placement, assistance with FMLA, housing, and other community supports.


Responsibilities include:

  • Assess potential patient's treatment needs, locating a time for patient to complete an intake assessment.
  • Create and maintain client files, ensure complete and timely documentation, and assist clients with completing any program documentation.
  • Effectively determine, communicate, and collect necessary finances due upon admission to the facility.
  • Maintain positive relationships with potential patients, family members, referents, and other community members.
  • Follow up with daily scheduled admissions and intakes.
  • Provide compassionate, knowledgeable customer service to a wide variety of individuals.
  • Excel in teamwork, working in unison with all staff to ensure positive patient experience.
  • Maintain active caseload providing intensive case management services throughout the course of a patient's treatment.
  • Work collaboratively with the treatment team to assist clients in determining continuing care placement.
  • Assist clients with locating sober living homes or extended care.
  • Assist with reintegration into the community and independent living.
  • Link clients with appropriate community resources and assist in implementing self
- directed service plans.

  • Provide a wide variety of client services such as counseling, referrals and resources, life skills training, employment assistance, and health maintenance.
  • Collaborate with outside treatment providers and other partner agencies.
  • Participate in meetings with service providers including physicians/psychiatrists to assist clients in achieving their goals.
  • Performs all other duties as assigned.

Minimum Qualifications:

EDUCATION/CREDENTIALS

  • Associates or Bachelor's degree in human service field with addiction credentials preferred.

EXPERIENCE

  • Prior experience working in an inpatient mental health/addiction treatment center preferred.
  • Experience working in a team setting.

KNOWLEDGE AND SKILLS

  • Understanding of 12 Step Recovery process
  • Proficiency in Microsoft Excel and Office
  • Ability to adapt and change focus to new tasks or goals.
  • Ability to negotiate or overcome barriers.
  • Attention to detail documentation skills.
  • Demonstrate excellent interpersonal communication skills.
  • Ability to motivate those seeking treatment.
  • Ability to connect and empathize with patients and families seeking treatment.


Mainspring Recovery conducts as needed, job-related background checks, fingerprints, drug testing, TB testing, verification of employment history and/or reference checks prior to employment.

Full-time, 40 hours per week, shift work 8:00am-4:30pm Monday-Friday, weekends may be required
