As a member of the Assertive Community Treatment (ACT) Team, the ACT Employment Specialist provides direct treatment, rehabilitative and supportive services in a community setting to consumers. Provides direct job development services and employment support services to assist to obtain and maintain community-based employment. Works as a team member of a clinical team to assess consumer needs and wants, participates in the development of individual service plans, acts as a consumer advocate and liaisons with families and community agencies. Provides on-call crisis intervention services, as well as weekend and holiday coverage. Flexible schedule required.
JOB SUMMARY (Including, but not limited to)
- Works as part of a clinical team to facilitate recovery and inspire hope for consumers while supporting their achievement of service plan goals. Provides ongoing assessment and feedback as to their progress in relation to their service plan, regardless of the consumer's current location.
- Works with consumer's Prescribing Clinician, Nurse, and other members of the clinical team to ensure the timely development and implementation of the individual's service plan (including all necessary assessments) and their crisis plan.
- Implement career and rehabilitation plans as directed by the team Rehabilitation Specialist and Clinical Coordinator.
- Provides direct job development to the consumers assigned to the team.
- Facilitated informational, educational, and supportive activities including Orientation into the World of Work, the effect of work and meaningful community activities on benefits programs, job seeking skills, job clubs and employment support groups.
- Provide ongoing monitoring and long-term support for consumers involved in meaningful activities, including employment.
- Responsible for transporting consumers to necessary services, as outlined in their service plan, as needed.
- Assumes "primary contact" responsibility for a percentage of team consumers to: assure maintenance of accurate, thorough, and current documentation in the medical record (in accordance with policies, procedures, and clinical protocols) for those specific individuals; keep weekly client schedule for those individuals updates, and act as first contact for those consumers and their families.
- To ensure compliance & adequate services, additional job duties may be required to meet the needs of the program and or department, consistent with Copa Health productivity levels
- High school diploma or high school equivalency diploma and four (4) years of full-time direct care experience in behavioral health or
- Bachelor's Degree in a related field OR a bachelor's degree in a non-related field with one (1) year experience preferred.
- Must have 1 year of training or experience in vocational rehabilitation and support. Prior experience may be job development, job coaching, or supported employment.
- Must have a Level One Fingerprint Clearance Card by the AZ Department of Public Safety or eligible to receive one within eight (8) weeks of hire.
- TB Test clearance required at the time of hire.
