Overview:

The Manager, Care Management- Social Work & Hospice Transitions (Manager) is responsible for managing the day-to-day operations of the Care Management hospital social work and hospice transitions teams. This include ensuring effective care coordination and collaboration with the interdisciplinary team, analyzing the departments use of internal and external resources in order to achieve optimal patient outcomes, promoting high-quality, cost-effective patient management, optimizing department resources, training team members, mentoring colleagues, and reviewing colleague performance. The Manager leads the team to work collaboratively with the medical team to meet the care management aspects of a patients illness and ensure resources are in place to support patients and caregivers.

Qualifications:

Education:

  • Master of Nursing degree from an accredited College of Nursing

OR

  • Master of Social Work degree from a School of Social Work accredited by the Council on Social Work Education

OR

  • A Master of Healthcare Administration, Business Administration, or related degree required.

Licensure/Certification/Registry:

  • Current licensure as a Registered Nurse (RN) in the State of Illinois required with Master of Nursing degree

OR

  • Current licensure as a licensed social worker (LSW) in the State of Illinois required with Master of Social work degree

Experience:

  • Management experience preferred.
  • Evidence of experiences, which define competence in personnel management and program development.
Responsibilities:
  1. Manages, implements and champions strategic change. Oversees the allocation and assignment of social work and hospice transitions teams for proper utilization of personnel in relation to patient, service line, and organizational needs. Manages the daily operational activities of all social work and hospice transitions team functions in conjunction with other members of the management team.
  2. Interviews, hires, trains, retains and continually educates all social work and hospice transitions colleagues. Develops a methodology to routinely measure, report and review individual performance and achievement of outlined indicators. Identifies opportunities for professional and personal development.
  3. Completes studies and analyses as needed to evaluate various operational issues and processes and present findings with appropriate recommendations. Analyzes data reports that contribute to improvements in patient care, discharge planning, social work/hospice transitions protocols and productivity, and hospital-related services. Collects patient statistics, volumes, costs, and outcome data for the department. Assists in enhancing the departments quality improvement and compliance initiatives.
  4. Provides expertise and direction to the social work and hospice transitions teams when resolving complex patient problems, discharge planning, continuity of care, and systems management. Develops, modifies and implements effective policies and procedures in order to meet operational goals and objectives.
  5. Assists System Director in developing a strategic plan for social work and hospice transitions that reflects the strategic planning priorities of Memorial and the changing health care environment. Develops personal, professional and team goals/objectives within the department and other affiliated hospital areas. Reviews social work and hospice transitions teams primary responsibilities and duties of the job in an attempt to improve the utilization management, discharge process and enhance efficiency.
  6. Facilitates relationships between care management, patient care facilitators, discharge planning specialists, physicians, nurses, and post-acute care providers. Coordinates with members of the healthcare team and other ancillary staff to obtain complete and accurate information regarding a patients medical and financial status.
  7. Serves as resource to clinical team, patients and families regarding entitlement to programs and support services.
  8. Develops strong working relationships with internal and external healthcare organizations and community resources.
  9. Promotes high quality, patient focused, cost effective patient management across the continuum of care for High-risk, complex, and resource intensive patient populations.
  10. Reviews vendor contract and service agreement as well as performance/compliance with contract terms.
  11. Performs other related work as required or requested.
