The meaning of the Case Manager:

As part of the Resident and Community Services division, working under the guidance of the Resident & Community Services Director, the Case Manager role is crucial for supporting operations at properties housing low-income, previously homeless, disabled, and mentally ill individuals. The focus of case management revolves around ensuring the stability and retention of housing for those in permanent supportive housing and others with special needs. This position involves providing comprehensive services to help residents adhere to lease agreements, build positive relationships within the community, access necessary resources, and develop plans for achieving housing goals while promoting individual self-sufficiency and mental well-being.

Key Responsibilities:

  • Developing and implementing housing plans and budgets for each program participant.
  • Coordinating with various internal and external systems to enhance housing outcomes for households.
  • Maintaining regular contact with housing clients and ensuring proper documentation and upkeep of records.
  • Providing trauma-informed care and motivational interviewing in case management.
  • Conducting assessments to identify housing needs and barriers, and connecting clients with appropriate resources.
  • Facilitating access to services supporting housing readiness and addressing barriers.
  • Collaborating with property management to assist clients in securing necessary documents for housing.
  • Assisting residents in applying for housing assistance programs.
  • Participating in supervision, agency, and community meetings as needed.

Skills, Knowledge, and Abilities Required:

  • Thorough understanding of social services, public assistance, and related systems.
  • Ability to show empathy and non-judgment when working with clients.
  • Effective advocacy for low-income and homeless populations.
  • Comfort working from a trauma-informed and strength-based perspective.
  • Adaptability and strong multitasking abilities in a fast-paced environment.
  • Familiarity with Trauma-Informed Care, Motivational Interviewing, and Harm Reduction Models.
  • Knowledge of fair housing laws and tenant relations.
  • Proficiency with Coordinated Entry System, HMIS/ICMS, and Microsoft Office Suite.
  • Preferred fluency in English and Spanish.

Education & Experience Requirements:
  • Bachelor's degree in social work or related field with 2 years of social services experience, OR
  • Master's degree in social work or related field with 1 year of demonstrated case management.

Licenses & Other Requirements:

A valid California driver's license and auto liability insurance are mandatory.
