Position Summary:

This role involves offering patient-focused social work services directly to patients and families within a hospital environment and playing a key role in the interdisciplinary healthcare team. The Social Work Care Manager will collaborate with inpatient and ambulatory care teams, patients, and their external support network (if applicable) to identify potential social work needs. It is important for the Social Work Care Manager to consistently apply social work values and strive for continuous quality improvement in their daily tasks.

Education/Experience:

Bachelor's Degree in Social Work from a program accredited by the Council on Social Work Education (CSWE), the Canadian Association of Schools of Social Work (CASSW), or a similarly recognized accreditation body approved by the Minnesota Board of Social Work is required. Previous experience in healthcare or social work is advantageous.

  • Capability to combine social and medical data for psychosocial assessments and implementation of social work treatment plans

Licensure: Licensed Social Worker (LSW) by the MN Board of Social Work

Additional Information:

The BSW/MSW Hospital Social Worker is responsible for coordinating discharge planning for patients at Methodist Hospital. The work hours are from 8 AM to late afternoon with a weekend rotation every sixth weekend. A Bachelor's or Master's degree in Social Work and a valid license are required. Experience in a hospital or healthcare environment is highly preferred.
