Your responsibilities will include conducting intake assessments, developing service plans, coordinating services, recording case notes according to standards, and ensuring quality care to enhance the well-being of our residents.
- Provide direct care services to tenants, focusing on activities of daily living skills such as budgeting, grooming, and nutritional planning
- Establish and maintain cooperative relationships with community agencies, schools, and medical facilities
- Monitor tenants' progress, coordinate evacuations in emergency situations, and plan recreational/social activities for residents
- Maintain and update tenant files, attend meetings, and participate in required trainings for continuous professional development
- Qualifications: High school diploma/associate degree in a related field, minimum of 2 years of experience in case management or social services, experience working with diverse populations is highly desirable
Additional Responsibilities:
- Participate in activities of other staff members in their absence or during staff shortages
- Represent the agency at meetings and trainings
- Ability to work a flexible schedule and drive facility vehicle when required
- Maintain confidentiality and perform any other duties assigned by the supervisor
