Under the direct supervision of the Director, Child & Adolescent Services, the LMHP will provide in-depth psycho-social assessments of residents who live in the 5 counties.

The LMHP will assess the need for psycho-metric testing as needed. This LMHP will receive assignments from the RYC EIR Management Team. The LMPH will ensure quality benchmarks and contract obligations are met.

The Lead will provide assessments, coordinate with area psychologist to secure psycho-metric testing as needed, and complete comprehensive reports and recommendations for treatment.


The LMPH will receive case assignment from the Director, Child & Adolescent Services and other RYC Crisis Contract Mgt Team members.

The LMPH will ensure that the care model and contract requirements are completed, and that best care and patient outcomes are provided for every encounter.

Provide support and education on crisis system services to all providers and the community in the Eastern Iowa Region. Build relationships with JCS, HHS, AEA, DeCat, School Districts, Behavioral Health Providers, First Responders, and other key stakeholders.

Assist the Children's Services Coordinator in developing, designing, and tracking metrics and researching outcomes of EIR services.

Responsibilities:
Program Level

Point of contact with the EI Region for individuals requiring comprehensive evaluation, testing, and care planning and development

Receive referrals from EIR Care Coordinators, for individuals experiencing the highest level of mental health needs

Coordinate assessment, evaluation and needed psychometric testing

Coordinate care planning among community agencies, schools, and families.

Coordinates Crisis Stabilization Planning as needed

Phone call made to referring agencies within 72 hours

Phone call made to youth/family within 72 hours

Completes/submits data collection

Community


Builds relationships with hospitals, community mental health centers, substance use disorder providers, JCS, HHS, AEA, DeCat, School Districts, First Responders, law enforcement, jail staff, Communications departments.

Provide community education on services available through the EIR Crisis System.

Qualifications:

Education:
Requires an Master's degree in social work, marriage and family therapy, or counseling.

Experience:
Minimum of five years' experience in mental health or human services.

License/Certification:
Valid driver's license when driving any vehicle for work-related reasons.

Must hold independent licensure status in Iowa.

Knowledge/Skills/Abilities:


Must demonstrate ability to assess and identify and act upon psychosocial needs and work independently in an outcome orientated environment.

Strong ability to connect and create a trusting environment with patients is required.

Strong leadership skills and ability to form professional working relationships with peers.

Must have interpersonal and communication skills necessary to deal with a wide variety of patients and professionals with the ability to negotiate and establish effective working relationships with a multidisciplinary healthcare and community service team.

Develop, design, and track metrics and researching outcomes.

Ability to document in, have knowledge of and use of the computer.
