Transitions Hospice is looking for a Chaplain to join their team. This role involves providing non-denominational spiritual care services to patients, families, and caregivers, either directly or by coordinating care with other spiritual counselors. Local travel is required for this position.

Qualifications

  • Graduate of an accredited seminary or school of theology, or hold a degree in counseling
  • Experience in working with death and dying, individual/family/caregivers
  • Comfortable in an interfaith setting
  • Ability to network with community clergy and congregations
  • Preferably, experience in hospice/home health
  • Valid driver's license and reliable transportation with auto insurance

Responsibilities

  • Assess spiritual needs of patients/families/caregivers and provide timely consultation
  • Coordinate with preferred spiritual advisors and clergy when needed
  • Develop and document spiritual care plans with the interdisciplinary team
  • Evaluate and revise care plans regularly
  • Maintain records of spiritual care services and related activities
  • Document interactions with local chaplains and spiritual counselors
  • Offer consultation, education, and support to the interdisciplinary team on spiritual care
  • Conduct pre-bereavement and bereavement assessments and contacts
  • Establish networks with faith communities and provide funeral or memorial services
  • Supervise spiritual care volunteers and manage caseload efficiently
  • Participate in evening/weekend on-call services
