Summary:


Working independently under the direction of a physician, AP or RN, the LPN participates in direct and indirect nursing care, health maintenance, teaching, counseling, collaborative planning and rehabilitation.

Obtains an individual's basic health status and related data, records data; participates in analyzing and interpreting patient data, and making informed judgments and incorporates the prescribed medical regimen into the nursing plan of care; participates in the health teaching required by the individual and family to maintain an optimal level of health care; when appropriate communicates outcomes of basic nursing intervention and proposes change in plans of care after consulting with physician, AP or RN; collaborate, cooperate and communicate with other health care providers to ensure quality and continuity of care.

LPN's at this level are seen as a go to person and demonstrate advanced knowledge in specialty area and a broad range of operational and clinical issues.


Job Responsibilities:
1)

Clinical:

Under the direction of a physician, AP or RN develops plan of care for patients for review and approval including recent advances and current knowledge in the field.

Provides timely, appropriate triage to ensure access to safe, effective, efficient care in the appropriate setting. Consistently demonstrates advanced knowledge in specialty area and resolving clinical issues. Demonstrates skill in dealing with difficult patients to resolve conflicts amiably. Consistently meets professional and organizational quality and safety standards. Is a CSIS resource and assists in developing/delivering education or communication on new functionality.

Clinical Practice Innovation:
Develops and incorporates creative and valid interventions into clinical practice. Supports and champions change. Makes recommendations for and takes lead in establishing and implementing protocols, treatment plans or programs using evidence based criteria. Promotes Patient Safety and uses SRS reporting to develop action plans. Recognizes patterns and issues that may present safety issues to patients and works with practice leadership to resolve. Participates in the development of educational materials for patients, groups of patients and/or staff. Works across organization to advance practice.

Nurse/Patient/Family Relationship:
The nurse establishes and maintains a healthy, balanced and therapeutic relationship with the patient/family. The nurse demonstrates respect, compassion, cultural competence and sensitivity and advocates for the patient's rights, values and belief systems. Evaluates barriers to access care.

Screens patient for unmet needs and considers the influence of social and cultural differences that might impede patient care and develops solutions.

Supports patient/family to be involved in the clinical decision making process with the team and providers. Establishes effective access to care within the practice and advocates for access with specialty providers, facilities and ancillaries. Demonstrates active listening and compassion combined with professional objectivity and nonjudgmental collaboration when working with patients and families.

Patient/Family Education and Health Care Transition Management:
Demonstrates a commitment to identifying and addressing the learning needs of both patients and families. Proactively seeks out opportunities to ensure a safe care transition between facilities and primary care. Identifies, anticipates and addresses the learning needs of both patients and families. Considers the patient's cognitive, physical and emotional readiness for processing information, making adjustments as needed. Anticipates and provides patient's resource and care needs when transitioning to another facility or to home. Provides outreach and transition management across the continuum of care. Develops networks and resources within BH and externally.

Health Care Team Relationships:

The nurse cooperatively interacts within the health care team to support and contribute to the shared patient centered/family focus care goals.

Effective and efficient use of technological and human resources to achieve desired patient/family outcomes.

Effectively interacts within the health care team to manage and coordinate the patient centered and family focused care along the continuum of care.

Demonstrates efficient use of resources to achieve optimal patient and family outcomes.

Optimizes each patient encounter and coordinate with providers, labs, pharmacy, interpreter, social services and health team members to ensure continuity, access, comprehensive care and patient satisfaction.

Acts as a resource for Baystate Health to train and facilitate team building relationships. Addresses and transforms difficult behaviors of team members.

Communication:

Communicates and coordinates with all entities involved in the care of a patient to promote and maximize care coordination and patient satisfaction.

Manages difficult conversations and resolves issues or recommends alternative solutions. Successfully deals with challenging interactions. Acts as a role model for effective communication across the organization. Delivers difficult information clearly in a positive manner.

Leadership:

Collaborates with the practice manager and medical director to create and maintain an environment that promotes the development and integration of visions and ideas.

Works within system to accomplish patient, practice departmental and institutional goals. Supports the staff development through coaching, mentoring and role modeling. Seeks opportunities to build working alliances with other professional groups. Supports and models the expectations for behavior and communication in the practice and holds individuals accountable for their actions. Acts as clinical lead when needed, ensuring that office, patient and provider needs are met.
