That's why we offer personal care, palliative care and hospice to give people the care they need with dignity, grace, and love. The Afterhours AIM RN will report directly to the Director of AIM. Responsible for identifying and coordinating patient/family needs and for providing preventive and supportive care to the palliative patients and family. Utilizes assessment, intervention, and teaching skills to maximize patient comfort and to enhance the quality of life. Assumes afterhours coverage for the designated teams and functions as an interdisciplinary advanced illness management and support, along with telephonic, video, and in person interventions afterhours.
