Practices as a Registered Nurse under the guidance of the Nurse Manager for Primary Care within Montana VA Health Care System and supports the mission and vision of the Healthcare System. Primary Care provides acute and chronic care needs for Montana VA Health Care System including a variety of services for adult and elderly Veterans. The PACT RN Care Manager uses the nursing process and evidence based practice to collaborate with the Teamlet (Patient, Primary Care Provider, RN Care Manager, LPN, and MSA) and larger team (family/caregiver, PCMHI, RD-CDE, SW, PharmD, internal and community based services involved in providing care to the patient) in developing the patient-driven holistic plan for life skills.

Duties include, but not limited too:
Responsible for providing competent, evidence-based gender-specific care to primary care patients and oversight of licensed vocational nurses/nursing assistants, as appropriate to the setting.
Provides direct Veteran care and care coordination, as it pertains, to Veterans and their care within the Primary Care Department.
Manages assigned panel(s), utilizes registries, and ensures appropriate evaluation and access is provided to patient panels.
Provides health promotion, health education and coaching on wellness, disease prevention, and chronic care management.
Assists in directing the provision of nursing education, orientation, competencies and providing quality improvement and outcomes utilization.
Demonstrates leadership in delivering and improving holistic, primary care through collaborative strategies with others.
Evaluates practice in an ongoing process, based on best evidence.
Provides peers with informal constructive feedback for improvement.
Fosters a safe and supportive environment conducive to the professional development of healthcare professionals.
Contributes professional outpatient nursing perspective in discussions with the interdisciplinary team.
Partners with others to effect change and produce optimal outcomes.
Supports colleagues through knowledge sharing to provide safe, quality outpatient nursing care.
Shares educational findings, experiences, and ideas with peers.
Questions PACT clinical practices for the purpose of providing evidence-based care.
Participates in activities and strategies to sustain an evidence-based-practice culture.
Responsible for collaborating with services internal and external to Montana VA Health Care Services to facilitate care transition in order to effectively meet the patients' needs and provide a interdisciplinary approach to care.
Provides patient and family health education with a focus on self-management, shared decision making, prevention, and wellness, based on the individual patient's goals.
Demonstrates fundamental knowledge by serving as an advocate for patients, team player to colleagues as s/he continues to enhance his/her own, and the team's professional growth, development, and practice.
Provides patient and family education with a focus on self-management based on the individual patient's goals.
Provides the Veteran with ongoing support and coaching needed to succeed in making and sustaining changes in their health through a personalized, proactive, patient centered plan of care.
Uses research based knowledge and guidelines, standardized nursing protocols, and professionally accepted practice standards to promote patient engagement, self managed care, shared decision making, and wellness to patients and to determine care management requirements for individual or cohorts of patients.
