Palliative Care Team currently consists of three (3) Physicians and four (4) Nurse Practitioners, Social Worker and ChaplainSchedule is Monday through Friday; 8am-5pmThis is an outpatient opportunity as we are looking to expand our outpatient role in the community.Will work collaboratively with physician, nurse practitioners, other consultants and primary team, as well as an interdisciplinary team including Social Worker, Case Management and chaplainWill participate in the initial consults, develop and implement careCommunity education and outreachCall is 1:5EMR Platform – Integrated Epic/EHR with HIMSS Stage 7 validationExceptional employed Hospitalist & Critical Care team, consisting of over 125 providers, offers 24/7 supportOpportunity to work with our Hospice and Palliative Care fellows**The ideal candidate will possess a mature, warm and patient personality with a passion for palliative medicine and care. Flexibility and compassionate bedside manner are essential. Must be a self-starter who takes initiative to work collaboratively with Interdisciplinary Team.
