Duties include, but not limited to:
Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care.
Assesses, plans, implements, and evaluates care based on age specific components.
Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Administers medications and procedures per established policies and guidelines.
Influences care outcomes by collaborating with members of the interdisciplinary team.
Executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex client care.
Responsible for coding the sections of the Resident Assessment Instrument (RAl)/MDS Assessment and the Care Area Assessments (CAAs) that are assigned to nursing, adhering to current Centers for Medicare and Medicaid Services (CMS) and Veterans Health Administration (VHA) timeframes of assessment completion.
Participates in the CLC Interdisciplinary Team care plan meetings and reviewing, communicating, and monitoring the residents' care plan.
Collaborates with the CLC Resident Assessment Coordinator (RAC).
Seeks documentation guidance from the RAC to ensure accuracy of the Resource Utilization Group (RUG) score. Supports quality improvement activities based on CLC quality management data.
Participates in ongoing survey readiness activities in the CLC.
Documents and takes corrective action when notified by the CLC management team, charge nurse, or designee of any discrepancies in coding or documentation of RAI/MDS assessments and CAAs reported during the interrater reliability review.

Yes, this is a compressed tour, 6 x 12 hour shifts and 1 x 8 hour shift every 2 weeks.
