Unit Charge Registered Nurse

** Competitive Pay: $41 - $46/hr. (Dependent on Experience) ** Work hours: Primarily during the day with some flexibility based on admissions needs.
In this role, your daily responsibilities may include:
- Collaborating with hospital case managers, patients, and their families to facilitate placements for skilled nursing services or rehabilitation therapy.
- Engaging with interdisciplinary team members to ensure seamless transitions from various healthcare settings to our Skilled Nursing Health Center.
- Overseeing inquiries and managing the referral process through our Referral Manager System to support the admissions team.
- Assisting the admissions counselor with inquiries from families, hospitals, assisted living facilities, and other referral sources.
- Maintaining strong communication and relationships with hospital accounts and accountable care organizations.
- Ensuring adherence to the standard admission process via our electronic health record system, managing incoming continuity of care documents.
- Effectively communicating the specific needs of new admissions to the interdisciplinary team and primary care providers to guarantee quality, person-centered care.
- Participating in care conferences, assisting with appointments, and fulfilling other nursing duties as required.
To excel in this position, you should possess:
- A valid RN license in the state of application.
- Current CPR certification.
- Previous nursing experience in senior care, skilled nursing, admissions, or post-acute care is preferred.
