The Lead HUD-VASH Case Manager, a professional social worker, functions as Eastern Kansas Health Care System's (EKHCS) Lead for Housing and Urban Development-VA Supportive Housing (HUD-VASH) Program. The HUD-V ASH program which is located at both the Topeka and Leavenworth campuses is a collaborative effort, supported through HUD Section 8 rental assistance vouchers and VA' s provision of intensive case management services. The primary goal of HUD-VASH is to move veterans and their families out of chronic homelessness. The primary component of the program is VA case management services, designed to improve veteran's health and mental health, enhancing their ability to remain stable, housed and community-integrated.
Due to the community housing focus of the HUD-VASH program, the work is to be done where the veteran resides, therefore, 70% or more of the HUD-VASH case manager time will be spent at a community location other than the VAMC or CBOC. The lead will not only promote effectiveness in the incumbent's social work function but oversee the effectiveness of other HUD-VASH staff. Veterans eligible for HUD-VASH come from a wide geographic area. Their health and mental health needs may be complex and require a high degree of clinical oversight and creative problem solving. Services are provided in healthcare settings and in the community.

Functions include but are not limited to:
Serves as the medical center's primary point of contact for the National HUD-VASH Program, Office of Mental Health Services, and VACO.
Responsible for data collection, administrative record keeping, and monitoring patient or program clinical indicators, thresholds, and performance measurements for the HUD-VASH program.
Develops working relationships and agreements with other organizations and directly coordinates with the designated PHA regarding housing voucher application and award process for each veteran.
Responsible for the significant program responsibility of developing a systematic program that operates as a unique panel of services in the area of permanent supportive housing.
Coordinates with other HUD-VASH case managers to verify that the services are able to be integrated into the overall Domiciliary Care for Homeless Veterans Program.
Accountable for full utilization of the vouchers in his/her catchment area which can be in excess of 100 miles in diameter.
Responsible for promoting effectiveness, evaluating and modifying services patterns in all areas of case management.
Oversees the determination of when veterans are eligible for HUD-VASH services as well as when they are no longer in need of case management.
Ensures that the organization's strategic plan, mission, vision and values are communicated to the HUD-VASH team and integrated into the HUD-VASH team's strategies, goals, objectives, work plans and work products and services.
Articulates and communicates to the HUD-VASH team the assignment, project, problem to be solved, actionable events, milestones, and/or program issues under review, and deadlines and time frames for completion.
Leads the HUD-VASH team in: identifying, distributing and balancing workload and tasks among employees in accordance with established workflow, skill level and/or occupational specialization; making adjustments to accomplish the workload in accordance with established priorities to ensure timely accomplishment of assigned HUD-VASH team tasks; and ensuring that each employee has an integral role in developing the final HUD-VASH team product.
Trains or arranges the training of HUD-VASH team members in methods and techniques of HUD-VASH team building and working in HUD-VASH teams to accomplish tasks or projects.
Monitors and reports on the status and progress of work, checking on work in progress and reviews completed work to see that the supervisor's instructions have been met.
Serves as a mentor to other mental health staff, particularly those at the entry level.
Provides training to students, residents, interns and fellows from other disciplines and conducts formal workshops and classes for staff.
