Summary

The Registered Nurse (Case Manager) with the VA Community Care (VACC) Nursing Service is directly responsible to the VACC Nurse Manager and is responsible and accountable for the coordination of care for a culturally diverse veteran population.



Duties

The RN (Case Manager) possesses the knowledge and skills to effectively apply all aspects of the nursing process and care management principles within a collaborative, interdisciplinary practice setting, which will enable effective implementation of health promotion and prevention practices, management of acute and chronic illnesses and attainment of the most effective level of care.
Duties include, but are not limited to:

  • Self-directed In assessing, planning, implementing, coordinating, monitoring and evaluating patient care activities in relation to the vendors within the Community.
  • Demonstrates effective coordination of care of Veterans requiring specialty care and testing at other VA hospitals within the network or through the VACC consultants.
  • Enhances understanding of the RN Case Manager for Veterans role through availability and interactions.
  • Maintains databases of all Veterans on all scheduled, cancelled, no show appointments and results and follow-up of all information pertaining to the VACC program; utilizes information to make changes in the services.
  • Maintains database of all Veterans to monitor the delivery of services and coordination of care and tracking patient outcomes.
  • Accurately documents in the medical record patient consult statuses and updates.
  • Uses a team approach to identify, analyze, and resolve patient care problems related to consult management that result in improved patient outcomes.
  • Demonstrates grade-appropriate and consistent critical thinking skills.
  • Perform other duties as assigned.
Work Schedule: Monday through Friday 7:30AM-4:00PM Subject to change based on agency needs.
