The Registered Nurse (RN) utilizes the nursing process to direct and deliver comprehensive nursing discharge instructions to patients in discharge suite in a safe and proficient manner according to individual patient needs. This position will assist in the coordination of post-hospital needs of all patients going home in order to promote quality, continuity of care and safety for the patient. The Discharge RN will meet the needs of the patient, to facilitate timely, and appropriate discharges from the hospital. Provides direct patient care based upon HCA and Las Palmas Del Sol Hospital Standards of Care and Standards of Practice, Polices, Procedures and Practice Guidelines. Provides guidance and supervision to Licensed Vocational Nurses (LVN), Nursing Technicians, Nursing Assistants and Unit Secretaries.
Obtains a nursing assessment history, and ensures proper inpatient care unit admission. Takes appropriate action when needed, such as communicating special circumstances to physicians and/or care team or reporting indications of abuse or neglect.
Completes and facilitates the completion of required documentation, such as admission assessments, risk assessments, medication reconciliation, discharge paperwork, and post-hospital documentation.
Teaches patients/families/caregivers about a patient’s medical condition, patient status, treatment plan, medications and possible side effects, and follow-up measures. Translates complex medical terminology to ensure complete knowledge.
Completes extra discharge documentation, education, and discharge care for special patient populations, to ensure compliance with core measures for discharge.
Ensures patients /families/caregivers receive copies of discharge paperwork and any other documentation needed, such as referrals for ongoing care, educational materials, etc.
Coordinates with House Supervisors/Nursing Administration to prioritize workload and identify greatest need areas to ensure smooth patient transitions.
Experience as a Patient Care Technician in an acute care setting or capstone rotation in St. David’s HealthCare and Las Palmas Del Sol facilities.
Associate’s Degree-Major: Nursing
Bachelor’s Degree-Major: BSN degree *
