**This is a full time position (30-35 hours/week) at our clinic located on Culebra Rd. Bilingual is preferred.**
*CVS Health is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to join our Primary Care team. In this capacity, you will work in collaboration with a dedicated clinical care team to provide evidenced-based care to a panel of patients. The clinical care team will work with the provider daily to drive better outcomes for entire panel of patients. This will include accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning. Patient-Centered Quality and Safety*

* Uses knowledge of etiologies, risk factors, pathophysiology, presentation, and epidemiology of medical conditions relevant to primary, acute, chronic, and transitional to provide care to patients
* inclusive of pregnancy prevention, STI Prevention/safer sex practices, contraceptive care counseling and medication management
* Counsels and educates patients and families on health maintenance and respond to patient care inquiries
* Evaluates and treats patients' medical needs with safe and high-quality treatment
* Documents all patient care within an electronic health record in accordance with current policies and procedures
* Works as a member of the clinical care team providing care and coordination of our patients with internal and external colleagues ensuring the highest level of care is always provided for all patients

* Provides patient-centered care that is compassionate, appropriate, and effective for the promotion of health, prevention of illness and treatment of disease

* Refers to appropriate specialists and patient care components as appropriate
* Collaborates with clinical care team to ensure the delivery of high quality, patient centric, longitudinal care
* Participates in and in some cases leads clinical care team huddles
* Demonstrates core values of CVS Health in all communications and interactions
* Communicates effectively with members of the clinical care team, other health professionals and health-related agencies.
* On call rotation shared with providers in market for after hours patient clinical needs or shift coverage
* Works effectively with clinical care team members

* Adapts quickly to new models of patient care for clinic efficiency

* Assist with hiring, development, and evaluation of Primary Care clinical care team as appropriate
* Complete necessary tasks for clinic operations, including but not limited to following up on lab results, and receiving incoming clinical information.
*Own the success of your practice through implementation of the Primary Care business plan designed to meet identified business goals
*Maintains self-awareness and professionalism of individual actions and how they impact the practice, and healthcare industry
* Continuing education, including what is required to maintain employment at CVS Health/ Primary Care, is the responsibility of the provider
*Or in select states, Master of Science in Physician Assistant Studies (or other health-related master’s degree or equivalent experience together with Bachelor of Science in Physician Assistant Studies) with current National Board Certification and State of Employment license to practice in the role
*A minimum of 1 year of recent Primary Care experience as a Nurse Practitioner in either a family medicine or primary care practice preferred. If not, successful completion of CVS Minute Clinic primary care training program.
* Working knowledge and understanding of quality measurement in the management of chronic disease conditions
* Ability to work remotely in a clinical care team culture
* Ability to work without direct supervision and practice autonomously
* Is proficient with information management and technology
* Capacity to collaborate with professional colleagues frequently to ensure quality patient care
* *While performing the duties of the job, the Primary Care NP/PA is regularly required to interact with patients remotely, sit, write, operate the computer and phone, speak intelligibly, and hear patient responses
* Specific vision abilities include the ability to view and read a computer screen and other electronic devices
*13This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.
