We are seeking a highly skilled and compassionate Family Nurse Practitioner to join our team at Affinity Family Care Clinic Pllc. As a key member of our healthcare team, you will provide high-quality primary care to our patients, working collaboratively with other healthcare professionals to deliver exceptional patient outcomes.

Responsibilities:
  • Conduct thorough health assessments and develop personalized treatment plans for patients.
  • Provide routine physical exams, manage acute illnesses, and offer basic wellness education to promote healthy lifestyles.
  • Maintain accurate and detailed patient records, ensuring seamless communication with other healthcare professionals as needed.
  • Perform minor office surgeries and procedures under the supervision of a licensed physician.
  • Manage care for patients with chronic conditions, such as diabetes and hypertension, to achieve positive outcomes.
  • Work collaboratively with our interdisciplinary team to provide comprehensive patient care.
Requirements:
  • 1 year of experience as a Family Nurse Practitioner in a primary care setting.
  • Certified Nurse Practitioner (CNP) licensure.
  • DEA License.
