Nurse Practitioner Position

CVS Health Careers is seeking a highly skilled and dedicated Nurse Practitioner or Physician Assistant to join our Primary Care team. As a key member of our clinical care team, you will provide evidenced-based care to a panel of patients, working collaboratively with a dedicated team to drive better outcomes.
Responsibilities- Provide patient-centered care, including accurate assessment, diagnosis, treatment, and management of health problems.
- Collaborate with the clinical care team to ensure the delivery of high-quality, patient-centric, longitudinal care.
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
- Participate in clinical care team huddles and contribute to the development of care plans.
- Manage clinical and non-clinical duties efficiently, prioritizing tasks and solving problems effectively.
- Adapt quickly to new models of patient care, ensuring clinic efficiency and quality patient outcomes.
- Enhance operational effectiveness, emphasizing cost containment without jeopardizing innovation or quality of patient care.
- Assist with hiring, development, and evaluation of Primary Care clinical care team members as needed.
- Master's degree level Family Nurse Practitioner program with current National Board Certification and State of Employment license to practice in the Advanced Practice role.
- Or, in select states, Master of Science in Physician Assistant Studies (or other health-related master's degree or equivalent experience together with Bachelor of Science in Physician Assistant Studies) with current National Board Certification and State of Employment license to practice in the role.
- Active Advance Practice Registered Nurse, Nurse Practitioner, National Board Certification or State of Employment license in good standing.
- Current National Board Certification, Family Practice.
- A minimum of 1 year of recent Primary Care experience as a Nurse Practitioner or Physician Assistant in either a family medicine or primary care practice preferred.
