JOB SUMMARY: This position has responsibility for supervising the daily operations of the department as well as oversight of all patient care services and staff. The supervisory functions of this position include ensuring quality patient care delivery in accordance with professional standards, laws/regulations, and quality initiatives within the department's budget.

JOB DUTIES:

Supervision

Facilitates the onboarding/orientation process for new employees
Provides ongoing positive and constructive feedback and coaching
Initiates coaching/performance improvement/disciplinary action in a timely manner
Coordinates performance evaluations in efforts to enhance employee performance and development
Promotes an environment that nurtures professional practice, encourages growth and helps ensure job satisfaction
Promotes cooperative working relationships with peers
Oversees staff competencies ensuring that staff are appropriately trained and maintain their skillset
Evaluates learning needs of staff and address learning gaps
Provides timely communication to leader related to patient care and service delivery issues
Addresses and communicates staff concerns, ideas and desired changes regarding standards, practice, procedure and policy development
Reviews clinical documentation and ensure completeness and appropriateness
Timecard oversight and approval
Manages overtime and productivity

Operations

Facilitates daily engagement system safety huddles
Oversees scheduling and work assignments, provides direction and makes adjustments based on patient needs, staff strengths and competencies
Assures the development, implementation and updates of the individualized plan of care
Reviews OASIS assessments for accuracy
Manages visit utilization
Manages staff communication and problem resolution to facilitate effective operations
Observes, listens and is responsive to what others communicate
Provides day-to-day coordination of care ensuring organizational policies and regulatory compliance & standards are maintained
Manages resources within the parameters defined in the budget
Assists in the development of new programs
Investigates and responds to client/family complaints
Investigates and responds to staff concerns/complaints
Works in conjunction with the Quality Compliance Coordinator to ensure regulatory compliance is met and maintained
Assists to identify opportunities for improvement and helps develop plans for improvement
Supports the QAPI program through audits, data gathering, education and role modeling
Participates in department activities
Provides care coordination and case manager support
Facilitates Interdisciplinary Team (IDT) activities
Implements and evaluates changes in practice, policies and procedures
Serves as a resource for the department
Oversees the referral/intake process to help ensure appropriate admissions
Works with providers and other clinical team members to coordinate patient transitions across the continuum of care to provide seamless transitions for the patient
Evaluates existing processes and problems in workflow looking for ways to improve and streamline practices
Practices stewardship by using, promoting and teaching effective and efficient use of resources

Professional conduct

Acts as a professional role model within the department
Participates in educational activities related to clinical knowledge and professional growth
Completes annual required learning
Maintains and applies current knowledge of professional/clinical practice
Promotes and produces high-quality work
Reports to work on time and completes scheduled shifts during assigned work hours

Maintains effective working relationships with all organizational staff and other as appropriate and necessary to accomplish the work requirements, programs and goals of the department/organization.

QUALIFICATIONS and CHARACTERISTICS:

Graduation from an accredited school of Registered Nursing, bachelor's degree preferred.
Must hold a current RN license in the State of Minnesota.
Minimum of five years nursing experience, at least 3 of which are in Home Care. Management or leadership experience at some level required, preferably in Home Care.
Demonstrated clinical competence and experience in Home Care.
Demonstrated competence and knowledge of Home Care regulations, reimbursement structure, quality data and benchmarks.
Must have the ability to function independently within the parameters of professional nursing practice, as shown by consistent clinical practice that produces appropriate patient outcomes and high patient-family satisfaction.
Must have proven critical thinking skills to objectively solve complex problems in rapidly changing patient care services.
Must possess effective communication skills with other staff, departments, agencies and patients and families.
Demonstrated ability to follow and adhere to Grand Itasca Clinic & Hospital policies, vision, mission and values.
Maintains reliable and consistent attendance.
