LPN - Clinical Support Specialist

POSITION SUMMARY:
The LPN Hospice Clinical Support Specialist is an office based position responsible for providing support and acting as a resource to clinicians in the field. You will assist in monitoring compliance, quality, and safe delivery of services by following a clinical process set forth by clinical management. This applicant must have a strong Hospice background with a minimum of two years of hospice clinic experience. Home Care Home Base experience a plus.

PRIMARY DUTIES AND RESPONSIBILITIES:
Operations: Serve as a resource for field staff, referral sources, physicians, patients and their families pertaining to clinical issues and/or concerns. Facilitate calls to and from Field staff; uses resources to resolve issues and reports anything of concern to manager. Assists with scheduling and clinical workflow. Assist in the evaluation of client needs for home health, appropriateness, and determining ability to accept referrals based on staff capacity. Supports agency quality and growth initiatives.
Patient Care: Work in the field with staff and patients as needed. Participates in on-call per agency needs.
Customer Service: Maintain positive working relationships with referral sources including physicians, hospitals, skilled facilities, assisted livings and families.
Compliance: Obtains physician orders as needed. Ensures compliance with other aspects of care delivery and documentation requirements, including consents, required notices and physician orders.
Policies: Maintain current knowledge of all Medicare/Federal/Insurance/Company guidelines, policies and protocol.
Teamwork: Assist with case conference meetings and development of in-services.
EDUCATION:
Graduate of an approved school of professional nursing
Licensed in the State of Ohio, in good standing to practice as a Licensed Practical Nurse
Assumes personal responsibility for meeting the continuing education requirements as set forth by the Ohio Board of Nursing
Current CPR Certification
TRAINING AND EXPERIENCE:
A minimum of two (2) years of experience as an LPN in a Hospice setting.
