Position Summary The Physician Assistant (PA) or Nurse Practitioner (NP) will work as a key member of our primary care team, delivering high-quality, patient-centered care. This role involves direct patient evaluation, diagnosis, treatment, and follow-up across a variety of acute and chronic conditions, with opportunities to contribute to practice development, patient education, and community health initiatives. Key Responsibilities Conduct comprehensive health assessments, physical exams, and preventive screenings. Diagnose and manage acute and chronic conditions in patients of all ages.- Order, interpret, and act on diagnostic tests and lab results. Prescribe medications and manage pharmacologic and non-pharmacologic treatments. Provide counseling and education to patients and families on health maintenance, disease prevention, and lifestyle modification. Collaborate closely with supervising physician(s), nursing staff, and administrative staff to ensure seamless patient care. Document encounters and care plans accurately in the electronic medical record (EMR). Participate in quality improvement projects and contribute to the development of practice protocols. Maintain compliance with all federal, state, and local regulatory standards, as well as ethical and professional practice guidelines. Support patient access initiatives, including same-day visits, telehealth services, and chronic care management. Qualifications Education & Licensure: Graduate of an accredited Physician Assistant or Nurse Practitioner program. Current licensure in the state of Vermont. National certification (NCCPA, AANP, or ANCC). DEA registration (or eligibility). Experience: Preferred: 1?2 years of clinical experience in family medicine, internal medicine, or urgent care. New graduates with strong clinical training and demonstrated commitment to primary care are encouraged to apply. Skills & Competencies: Strong diagnostic and clinical decision-making skills. Excellent interpersonal and communication abilities. Proficiency with EMR systems and comfort with telemedicine platforms. Ability to work independently while maintaining a collaborative team approach. Commitment to patient-centered, culturally sensitive care.
