We are currently looking for a Full-Time Field CareManager RN Coach to join the amazing team at our Employer Health Center located in Alpharetta, GA. 

 

Full Time, M-F 8-4:30

 

 

Essential Functions:

 

  • Develops personalized treatmentplans using provider care plans and member preferences using a coach approachand the pillars of Lifestyle Medicine  
  • Supports a proactive,multidisciplinary team approach directed toward prevention, education, andhealth promotion  
  • Provides condition specificself-care education to members enrolled in the various condition managementprograms  
  • Participates inoutreach/follow-up phone calls and follow-up member visits  
  • Documents touchpoints andprogress into the electronic medical record  
  • Collaborates with the full scopeof clinical team members to arrange for further medical treatment whenneeded  
  • Strives to work with member toimprove health outcomes and better management of any chroniccondition  
  • Delivers one on one and/or groupcoaching sessions in the areas of weight, nutrition, exercise, stress, tobaccocessation, work/life balance and general health  
  • Works closely with client’svendors as appropriate to support member’s needs where appropriate  
  • Identifies emergency situationsand provides care within legal scope of practice  
  • Acts as facilitator of Care Teammeetings.  
  • Coach effectively in-person andby phone  
  • May coordinate and conductwellness programs in areas such as nutrition, stress management, weightmanagement, smoking cessation, exercise and others  

  

Job Requirements: 

Education:

 

  • Currently licensed RegisteredNurse in state of practice  
  • Current Wellness CoachCertification from Wellcoaches® OR equivalent International CoachFederation-accredited program certification (must be obtained within 24 monthsof hire)  
  • Current hands on training in AHAor ARC Basic Life Support for health care providers required; Advanced CardiacLife Support may also be required based on contract scope ofservice.  
  • Bachelor's degree (BSN) fromfour-year college or university (preferred)  
  • Compact RN license preferred

 

Experience:

 

  • At least three years’ experienceas a Registered Nurse caring for members with any of the following conditions:Type 2 diabetes, asthma, heart disease, heart failure orhypertension.  
  • Experience working as a wellnesscoach  
  • Experience in ConditionManagement, Certificate in Case Management, Care Coordination, CertifiedDiabetes Educator (CDE) Certified Diabetes Care and Education Specialist(CDCES) a plus (preferred)  
  • Experience in health promotion orwork site wellness and public speaking (preferred)  
  • Experience with ElectronicMedical Records (preferred)  

 

Knowledge andSkills:

 

  • Knowledge with Microsoft Officeproducts (Outlook, Word, Excel, PowerPoint).
