Purpose of Position - Serves as a personal advocate for residents with intellectual and developmental disabilities (IDD) in the house. Coordinates and documents services received to ensure fulfillment of Person Centered Support Plans. Provides for the health, safety and welfare of the residents in their homes and in the community. Implements residents’ Person Centered Support Plans. Monitors and reports on data to ensure compliance with all applicable regulating agencies. Maintains professional relationships with residents as well as members of treatment teams. Performs all of the duties and responsibilities of a Residential Counselor. Serves as a personal advocate for one or more residents with disabilities in their home and community.Assists in managing the resident’s daily routine and in making their needs known. In coordination with assigned nurse, ensures resident's medical appointments are current. Schedules resident's medical appointments. Accompanies resident to medical appointment or, if unable to accompany themselves, arranges for other staff to do so. Monitors and reconciles personal needs ledgers weekly, while arranging unusual expenditures with Residential Site Supervisor. Serves an instrumental role in planning, writing, implementing, and tracking the resident’s Person Centered Support Plan. Helps to coordinate and attend Annual Service Plan Meetings 2. Provides for the health, safety, and welfare of the residents with disabilities in their homes and in the community. Attends to resident health and safety needs according to all assessment and care plans, including medication administration, personal hygiene, and home and community safety. Performs cleaning, cooking, and other site maintenance as assigned. Provides transportation as required using company vehicles. Ensures the ongoing facilitiation of person-centered principals in the home. Deals effectively with crisis situations. Supports individuals new to permanent supportive housing navigate systems and ensure long-term success. 3. Implements, monitors, and reports on residents’ plans and protocols as stated in their Person Centered Support Plan. Completes all documentation and tracking timely and accurately. Communicates effectively with residents, guardians and family members, and all other members of residents’ treatment team. 4. Monitors and reports on data to ensure compliance with all applicable guidelines and regulations, including Imagine Policies and Procedures. Completes all required training in the appropriate timeframe. Reports all behavioral incidents, medical emergencies, and/or significant changes in medical condition to Residential Site Supervisor, and completes all related paperwork in a timely manner. Completes and documents home visits in a timely manner for residential sites Maintains respectful and professional relationships with residents, staff, guardians and family members, and all other members of treatment teams. Confronts conflict productively, as described in the residential training. Other Duties/Responsibilities Strictly complies with household budget protocols and spending limits, including guidelines for resident personal needs funds. Main tains the residential site, vehicles, and resident personal belongings in good repair, being mindful of safety and cleanliness. Job Qualifications Knowledge, Skill, and Ability: General knowledge of the developmental disabilities system in Colorado. Demonstrated skill in working with adults with developmental disabilities, preferably in a residential setting. Effective written and verbal communication skills with individuals and groups at all professional levels. Ability to productively confront conflict. Demonstrated organizational and time management skills. Ability to work effectively with various technology including word processing software, internet-based databases, assistive technology, etc. Possession of a valid driver’s license and ability to meet Imagine driving requirements. Training/Education: High school diploma or equivalent. 2 years of college preferred. 3 months experience at the specific residential site preferred. Ability and willingness to successfully complete required training curriculum. Experience: 2 years of experience with people with IDD preferred. Experience with individuals with Brain Injury and substance abuse and/or willingness to learn on the job OR some combination of education and experience. Working Environment/Physical Activities Ability to lift 50 pounds. Ability to drive a van with wheelchair lift. Effective with shifting roles, responsibilities, and expectations in a changing environment. Ability to work various shifts, including overnight shifts. Willingness and ability to respond to emergency situations as they arise.
